Popular Vitamin B3 Supplement Raises Cancer Questions

Nicotinamide riboside, commonly called NR, has become a popular dietary supplement among people interested in energy, metabolism and healthy aging.

It is related to vitamin B3 and is sold without a prescription in many places.

NR has attracted attention because the body can use it to make NAD+, a molecule that cells need for many basic functions. NAD+ helps cells release energy from food and is also involved in DNA repair and other processes that keep cells functioning.

Levels of NAD+ can change with age and disease, leading scientists to investigate whether increasing it could have health benefits. This research has helped create a market for supplements that promise to raise NAD+ levels.

However, biology is rarely as simple as “more is better.” Research from the University of Missouri raised questions about whether increasing NR could have unwanted effects in some cancers.

The study was led by Elena Goun and published in the journal Biosensors and Bioelectronics. The researchers developed imaging methods that allowed them to follow NR and related metabolic activity in biological systems.

One focus was triple-negative breast cancer. This is an aggressive form of breast cancer that lacks three common features doctors often target with medicines: estrogen receptors, progesterone receptors and a protein called HER2.

Because those targets are missing, several hormone and targeted treatments used for other breast cancers do not work against triple-negative disease. Treatment commonly relies on approaches such as chemotherapy, immunotherapy and other therapies chosen for the individual patient.

Cancer cells need large amounts of energy and raw materials because they divide rapidly. They can change the way they process nutrients to support this growth, which is why cancer researchers are interested in metabolism.

The Missouri researchers investigated whether NR could influence these processes. Their findings suggested that high NR availability may support metabolic activity associated with tumor growth in the experimental models they studied.

The researchers also reported concerns about cancer spread, including spread to the brain. Metastasis occurs when cancer cells leave the original tumor, travel through the body and establish new tumors in other organs.

Brain metastases are particularly serious because they can be difficult to treat. The brain is protected by a specialized barrier that limits the entry of many substances, including some cancer medicines.

These findings sound alarming, but they require careful interpretation. The study does not show that taking an NR supplement causes breast cancer in healthy people.

Much of the research involved laboratory and animal models designed to reveal biological mechanisms. Results from these models can identify possible risks and guide further research, but they do not automatically predict what will happen in people taking ordinary supplement doses.

The amount of NR reaching tissues, a person’s health, the presence of an existing tumor and many other biological factors could change the effect. Researchers therefore need human studies before they can determine whether NR supplements meaningfully alter cancer risk or outcomes in patients.

The study nevertheless raises an important question about supplements that change metabolism. A substance that supports normal cells could potentially also be used by abnormal cells under certain conditions.

This principle is not unique to NR. Cancer cells are still human cells, so many nutrients and biological pathways needed by healthy tissue can also be used by tumors.

That makes claims about “boosting cellular energy” more complicated than they may sound in advertising. Increasing a useful molecule in the body does not guarantee that every effect will be beneficial.

The research may also have value beyond supplement safety. By tracking how NR is used, scientists may learn more about the metabolic weaknesses of cancer cells and eventually identify ways to interfere with the pathways tumors depend on.

This could contribute to more personalized cancer treatment. Tumors that look similar under a microscope can have different genetic and metabolic features, so understanding what an individual cancer uses to grow may help researchers design more precise therapies.

The study also highlights a broader issue with dietary supplements. Products can become popular before scientists fully understand their long-term effects in different groups of people.

“Natural” and “vitamin-related” do not necessarily mean risk-free, especially when a product is taken in concentrated amounts that may be much higher than those normally obtained from food. Supplements can also interact with medicines or existing health conditions.

For people currently taking NR, the research is not a reason to assume that cancer will develop or to panic. It is evidence that the biological effects of NR deserve further study, particularly in people who have cancer or may be at increased risk.

Anyone being treated for cancer should discuss supplements with their oncology team because even seemingly harmless products can potentially affect treatment or metabolism. People with other medical conditions can also ask a doctor or pharmacist whether a supplement is appropriate for them.

Overall, the Biosensors and Bioelectronics study provides an important warning against assuming that raising NAD+ is universally beneficial. It identifies a possible cancer-related concern, but it does not establish that NR causes cancer in humans.

The next step is careful clinical research that measures real-world exposure and outcomes in people. Until those data are available, NR remains an interesting but biologically complex supplement whose benefits and risks should not be reduced to simple anti-aging claims.
