To understand the impact of cuff size on modern automated blood pressure monitors, researchers studied 165 adults, averaging 55 years in age.
The majority (68%) of the participants were Black adults, with a one-third male demographic.
Each participant’s blood pressure was measured using both a regular adult-sized cuff and one appropriately sized for their arm. The findings were quite telling:
For individuals needing a small-sized cuff, the regular cuff gave significantly lower readings.
Conversely, for those requiring a large or extra-large cuff, the regular cuff recorded much higher readings.
Such inaccuracies have severe consequences. For example, those needing an extra-large cuff but measured with a regular one were wrongly classified as having a more severe stage of high blood pressure.
Meanwhile, individuals requiring a small cuff but measured with a regular one were incorrectly identified as not having high blood pressure.
These discrepancies mean patients could either be over-treated or not treated at all.
Who is Most at Risk?
The study highlighted a concerning trend: those needing larger cuff sizes are more susceptible to being misclassified in clinical settings.
Such findings are even more pertinent for communities with a high obesity prevalence, emphasizing the need for correct cuff sizing.

For accurate blood pressure readings – whether at a doctor’s office, a public kiosk, or in the comfort of one’s home – cuff size can’t be overlooked. Medical professionals and the general public must recognize and prioritize this.
