Why a Painful UTI Makes You Urinate So Often

The constant urge to urinate during a urinary tract infection can feel like one of the most frustrating parts of the illness.

New Australian research suggests that this reaction may not simply be a side effect of an irritated bladder. Specialized nerves may deliberately help trigger frequent urination as the body tries to remove harmful bacteria.

Scientists at Flinders University discovered that a little-understood group of nerves near the bladder lining becomes highly active during infection.

Their findings were published in Proceedings of the National Academy of Sciences. The discovery gives researchers a new view of how the bladder recognizes danger and responds to it.

The bladder is much more than a simple bag that stores urine. Its muscles, lining, brain connections, and sensory nerves constantly work together. As the bladder fills, nerves normally report changes in pressure and stretching so the brain knows when it is time to urinate.

For almost two decades, another group of nerves near the bladder’s inner surface has been harder to explain. These nerves, sometimes described as mucosal sensory nerves, do not respond strongly to the ordinary filling and emptying cycle. Their apparent silence made their purpose unclear.

Dr. Cindy Tay, Dr. Luke Grundy, and their colleagues investigated whether these nerves might have a different job. The team developed a new method that allowed them to study this nerve population more selectively in mice. What they found was a striking change when infection appeared.

In a healthy bladder, the nerves were mostly quiet. During a urinary tract infection, they became much more sensitive and reacted to bacteria and the inflammation surrounding the infection. Instead of acting mainly as sensors of bladder fullness, they appeared to act as sensors of biological danger.

This could explain several classic UTI symptoms. When the nerves become active, they may contribute to pain and the powerful urge to urinate repeatedly. Although these reactions are unpleasant, they can encourage behavior that may help the body defend itself.

Every time the bladder empties, urine can carry some bacteria out of the body. Increasing the number of times a person urinates may therefore make it harder for bacteria to remain in the bladder and grow. The nerve response may be one part of a larger defense system involving the immune system and the bladder lining.

Understanding this system matters because UTIs are extraordinarily common. They affect hundreds of millions of people globally each year, and many women experience at least one infection during their lives. Some people develop repeated infections, which can interfere with sleep, work, travel, and everyday activities.

A typical bladder infection may cause burning during urination, lower abdominal discomfort, frequent urination, and an urgent need to reach a toilet. If bacteria move into the kidneys, symptoms can become much more serious and may include fever, back or side pain, nausea, and feeling very unwell. Kidney infections require prompt medical attention.

The new findings may also offer clues about people who have problems with the nerves controlling their bladder. Spinal cord injuries and some neurological conditions can disrupt normal bladder sensation and emptying. If infection-sensing nerves are damaged as well, the body might have more difficulty recognizing bacteria and mounting the usual protective response.

The discovery creates a difficult treatment question. Scientists would like to reduce the pain and urgency caused by UTIs, especially for people with severe or repeated symptoms. But a medicine that completely silences these nerves might also remove signals that help the body clear bacteria.

Future therapies may therefore need to be selective. Researchers could try to identify which nerve signals mainly produce excessive pain and which are necessary for useful protective responses. The long-term goal would be to make patients more comfortable without weakening the bladder’s ability to defend itself.

The study is especially interesting because it gives a purpose to nerves that had remained mysterious for years. Its experimental method allowed researchers to isolate their behavior and show how dramatically it changes during infection. Publishing the work in Proceedings of the National Academy of Sciences also makes the findings available for further testing by other research groups.

Still, the results are early. The experiments relied heavily on mouse models, so researchers must confirm how closely the same nerve system operates in people. The work also does not show that manipulating these nerves can safely prevent or treat UTIs in patients.

The research nevertheless adds an important piece to the UTI puzzle. It suggests that the bladder has its own nerve-based warning system that works alongside immune defenses to detect infection and change behavior. What feels like an irritating need to keep visiting the toilet may, at least partly, be the body trying to protect itself.

If future studies confirm the mechanism in humans, these nerves could become a new target for treatments for bladder pain and urgency. The challenge will be to quiet unnecessary suffering without disabling an alarm system that may be helping to keep infection under control.
