Hidden Hip Disease Can Destroy the Joint if Missed

Persistent hip pain is often blamed on arthritis, especially in middle-aged and older adults. However, not every painful hip is caused by worn cartilage.

A review led by researchers at Yale University highlights three serious conditions that can cause hip pain even when arthritis is not present.

The review was published in EFORT Open Reviews and explains why these conditions are frequently missed during the early stages. Because they can permanently damage the hip joint, recognizing them quickly is extremely important.

All three conditions affect the femoral head, the rounded top of the thigh bone that fits into the hip socket.

This joint allows people to walk, climb stairs, bend, and remain active. If the bone becomes weak or collapses, pain and disability can become severe, and many patients eventually need a total hip replacement. Early diagnosis offers the best chance of preserving the natural joint.

One condition is osteonecrosis of the femoral head. It develops when blood flow to the bone is reduced or completely blocked.

Bone cells need oxygen and nutrients carried by blood, so without an adequate supply they begin to die. Over time the weakened bone can collapse under normal body weight.

The condition is often linked to long-term steroid treatment, heavy alcohol use, certain blood vessel disorders, and some metabolic diseases, although injuries such as hip dislocation can also trigger it.

A second condition is a femoral neck stress fracture. These fractures are tiny cracks caused when the bone cannot repair itself as quickly as damage occurs. Healthy athletes, runners, and military personnel may develop fatigue fractures after repeated heavy loading, while older adults with osteoporosis or poor nutrition can develop insufficiency fractures during normal daily activities.

Some fractures remain stable, while others can suddenly move out of place and require emergency surgery.

The third condition is called a subchondral insufficiency fracture. It develops just beneath the cartilage covering the joint surface. This problem mainly affects older adults, especially postmenopausal women with reduced bone strength.

Unlike arthritis, the pain may begin suddenly during an ordinary activity such as walking or after a small stumble that would not normally cause an injury.

The review explains that these three disorders often produce similar symptoms. Patients usually describe deep pain in the groin or hip that becomes worse when standing or walking. Because the symptoms overlap, doctors cannot depend on symptoms alone to identify the exact cause. A careful medical history and knowledge of risk factors are essential.

Another major challenge is imaging. Standard X-rays often appear normal during the early stages even when serious damage is already developing inside the bone.

Professor Daniel Wiznia and colleagues emphasize that relying only on X-rays can delay treatment. MRI is considered the best imaging method because it can reveal hidden bone damage before collapse occurs. CT scans can also help when MRI cannot be performed.

Treatment depends entirely on the diagnosis. Early osteonecrosis may be treated with procedures such as core decompression to encourage new blood vessel growth and reduce pressure inside the bone. Stable stress fractures may heal with restricted weight bearing, while unstable fractures often require surgery with screws.

Subchondral insufficiency fractures are usually treated by reducing weight bearing, controlling pain, and improving bone strength, although severe collapse may eventually require hip replacement.

The review reminds both doctors and patients that persistent hip pain should never be ignored simply because an X-ray looks normal. Modern imaging and early specialist assessment can prevent permanent damage and allow many people to keep their own hip joint.

The research published in EFORT Open Reviews highlights the importance of recognizing these conditions before irreversible collapse develops.

The findings have important clinical value because they encourage earlier investigation instead of waiting for symptoms to worsen.

Although this review summarizes existing evidence rather than testing a new treatment, it provides practical guidance that could reduce delayed diagnosis and unnecessary disability. Wider awareness among healthcare professionals and patients could improve long-term mobility and reduce the number of people needing hip replacement surgery.
