
For decades, doctors have searched for a simple way to tell who is most likely to develop Alzheimer’s disease before memory loss begins.
Most current tests either detect the disease after symptoms appear or require expensive brain scans or spinal fluid tests. A new blood test may eventually provide a much easier option.
Researchers from Mass General Brigham investigated whether a blood marker called p-tau217 could predict future Alzheimer’s symptoms in healthy older adults.
Their findings were published in JAMA and presented at the Alzheimer’s Association International Conference in London. The study suggests the test may identify people who are at particularly high risk many years before dementia develops.
The research included 2,684 adults who had normal memory and thinking when they entered long-running Alzheimer’s studies.
Each participant received a p-tau217 blood test and then returned regularly for cognitive assessments. During follow-up, nearly 500 participants developed cognitive impairment.
The scientists found a clear pattern. People with the highest p-tau217 levels faced a much greater risk of developing cognitive problems than those with the lowest levels. Those with very high readings had about a 38% chance of developing symptoms within five years and a 78% chance within 10 years.
Alzheimer’s disease is characterized by the buildup of amyloid plaques and tau tangles inside the brain. These abnormal proteins slowly damage nerve cells over many years before symptoms become obvious. The p-tau217 blood test appears to reflect this hidden disease process, giving researchers a window into changes taking place long before memory loss starts.
Despite these encouraging findings, researchers urge caution. Dr. Reisa Sperling said healthy people should not rush to seek this blood test because there is currently no proven treatment that changes medical care based on the results. For now, the advice remains to exercise regularly, eat a healthy diet, sleep well, and stay mentally and socially active.
Other Alzheimer’s experts welcomed the findings but noted that predicting dementia is extremely complicated. Some people accumulate amyloid plaques without ever developing dementia, while others experience different forms of cognitive decline caused by blood vessel disease or other conditions. These factors make accurate prediction difficult.
The researchers believe the immediate benefit of the test is for clinical research rather than everyday healthcare. By identifying volunteers who are most likely to develop Alzheimer’s, scientists can perform prevention trials more efficiently and determine whether new medicines work before symptoms begin.
The study also highlights how quickly Alzheimer’s research is changing. Blood tests are becoming increasingly accurate and may eventually replace more invasive or expensive methods in many situations. However, further validation in larger and more diverse populations will be essential.
Overall, this research represents an important advance but not a finished clinical tool. The study was well designed and included a large number of participants, yet experts agree that the test is not ready to provide personalized predictions for healthy individuals.
