
Blood pressure usually rises from childhood through middle age, and high blood pressure becomes increasingly common as people grow older.
But researchers have found that the pattern may change in later life. In many older adults, blood pressure begins a slow decline years before death.
Researchers examined medical records from 46,634 people in Britain who died at age 60 or older. By looking back through years of blood pressure readings, the team could track how the measurements changed as people approached the end of life. The decline began roughly 14 years before death and became more noticeable during the final years.
Blood pressure measures the force of blood pushing against the walls of the arteries. Persistently high pressure can damage blood vessels and increase the risk of stroke, heart attack, kidney disease and other health problems. This is why doctors often prescribe medicines and recommend lifestyle changes to bring high blood pressure under control.
Scientists had previously noticed that blood pressure can fall in very old people, but the reason was uncertain. One possible explanation was that more aggressive treatment of high blood pressure was lowering the readings. Another was that serious illness was responsible for much of the decline.
The study found that falling blood pressure was particularly strong among people with dementia, heart failure or major weight loss late in life. These conditions can affect the body’s ability to maintain normal blood pressure. People who had previously been diagnosed with high blood pressure also tended to show a substantial decline.
However, illness did not explain the whole pattern. The researchers found that blood pressure also declined in older people who did not have major conditions such as dementia or heart failure. This suggests that falling blood pressure may be part of a broader change that occurs during later aging.
The findings do not mean that lower blood pressure is necessarily harmful or that a decline predicts exactly when someone will die. The study identified a population-level pattern rather than a simple rule that can be applied to every person. An individual’s blood pressure can change for many reasons, including medicines, hydration, weight, heart function and other health conditions.
The results could nevertheless be useful for doctors caring for older adults. A blood pressure reading that was appropriate for someone at age 65 may need to be considered differently when that person is much older, frailer or living with several medical conditions. Doctors may need to look at the person’s overall health rather than focusing only on reaching a standard blood pressure target.
Importantly, older adults should not stop blood pressure medicines because of these findings. High blood pressure remains a major cause of cardiovascular disease, and treatment can reduce the risk of serious complications. Any change in medication should be made with medical advice after considering the person’s current blood pressure, symptoms and overall health.
The research also raises questions about why blood pressure falls so many years before death. Changes in body weight, blood vessels, heart function, the nervous system and other parts of the aging body could all play a role. Further research is needed to determine which changes are normal and when falling blood pressure might signal an underlying health problem.
For patients and families, the study supports careful monitoring rather than alarm. A gradual fall in blood pressure in later life should be considered alongside changes in weight, medicines, physical function and medical conditions. This more complete picture can help doctors decide whether treatment needs to be adjusted.
The research was published in JAMA Internal Medicine. By showing that blood pressure can follow a long downward path in later life, the study adds an important piece to scientists’ understanding of aging and may help improve the way blood pressure is managed in older adults.
