
For years, exercise has been promoted as one of the best ways to protect the brain as people grow older.
But a large new study suggests that the reason people are physically active may matter almost as much as how much they move.
Researchers at the University of Southern California found a striking difference between exercise done during free time and physical activity performed at work.
Leisure activities such as walking, cycling, running, swimming and sports were linked to a lower risk of dementia, while physically demanding work was linked to a higher risk.
Dementia is a general term for serious problems with memory, thinking and everyday abilities.
Alzheimer’s disease is the most common form, while vascular dementia is caused by problems with blood flow to the brain.
Regular exercise has long been associated with better brain health. It can improve heart and blood vessel health, help control blood pressure and blood sugar, and support other processes that may protect the brain.
However, earlier research often placed many different kinds of movement into one broad category called physical activity. This can include jogging in a park, cleaning the house, cycling to work or lifting heavy objects during an eight-hour shift.
The USC team wanted to know whether these very different activities were associated with dementia in the same way. They combined evidence from 74 studies involving more than 4.2 million people between the ages of 45 and 93.
The participants came from 30 countries and were followed for a typical period of about 10 years. During that time, researchers recorded cases of dementia, including Alzheimer’s disease and vascular dementia.
The clearest benefit was seen with exercise during leisure time. People with higher levels of recreational physical activity had a 24% lower risk of dementia compared with those who were least active.
The researchers found a very different pattern for activity at work. Higher levels of occupational physical activity were associated with a 20% higher risk of dementia.
This surprising difference is sometimes called the physical activity paradox. Similar findings have appeared in heart research, where physically demanding jobs do not always provide the same health benefits as voluntary exercise.
One explanation may be that leisure exercise usually gives people more control. Someone taking a walk or swimming can choose the pace, take a break when tired and stop when necessary.
Recreational activity may also be enjoyable and relaxing. It can involve spending time outdoors or exercising with friends, adding social and emotional benefits that may also support healthy aging.
A demanding job can be very different. Workers may spend many hours standing, lifting, bending or repeating the same movements while having little control over the speed or timing of their work.
Recovery time may also be limited. Repeated physical strain without enough rest could affect the body differently from a shorter period of voluntary exercise followed by recovery.
Work conditions could be even more important than the physical movement itself. Manual workers may experience greater stress, less control over their jobs and more exposure to noise, air pollution or other workplace hazards.
People doing physically demanding work may also be too tired to exercise recreationally after their shift. Differences in income, education, access to health care and other social conditions could further influence long-term dementia risk.
The study found limited evidence about other forms of activity. Household physical activity appeared to be linked to lower dementia risk, but this finding came from only one study, so firm conclusions cannot be made.
Active commuting, such as walking or cycling to work, was associated with a small increase in risk. However, only two studies provided evidence on this type of activity, making the result uncertain.
The researchers stressed that their findings do not mean people should avoid physically active jobs. The studies were observational, so they can identify patterns but cannot prove that physical work directly causes dementia.
Another limitation is that most participants reported their own activity levels. People may not accurately remember how much time they spend exercising or how physically demanding their work is.
Most of the research also came from wealthier countries. More studies are needed in lower- and middle-income countries, where dementia cases are expected to rise substantially as populations grow older.
Despite these limitations, the results remained fairly consistent when the researchers tested the data in different ways. This suggests that the difference between recreational and occupational activity deserves serious attention.
The study was led by Professor David Raichlen of the USC Dornsife College of Letters, Arts and Sciences, with Natan Feter as first author. The research was published in The Lancet Public Health.
Overall, the findings do not weaken the advice to remain physically active. Instead, they suggest that enjoyable exercise with personal control and adequate recovery may offer benefits that demanding physical work does not automatically provide.
The reported 24% lower risk and 20% higher risk describe associations across large groups rather than what will happen to any individual. Future research will need to separate the effects of movement from stress, working conditions, education, income and other factors that differ between jobs.
The study adds an important idea to dementia prevention: simply telling everyone to move more may be too simple. Safe places to exercise, enough leisure time and healthier working conditions could all matter when trying to protect the brain over a lifetime.
