
For years, researchers have noticed an unusual pattern: people who drink coffee often appear less likely to develop serious liver disease.
A major new study has strengthened that connection by following hundreds of thousands of adults and finding signs of better liver health not only in medical records, but also in liver scans and blood tests.
The study was led by scientists at Cedars-Sinai Health Sciences University and appeared in the journal Clinical Gastroenterology and Hepatology. Researchers examined data from 354,957 UK Biobank participants who were free of cirrhosis and liver cancer when the research began.
The participants were followed for about 13 years on average. During that time, the researchers used linked health records to identify people who developed cirrhosis or liver cancer and those who died from liver-related causes.
The liver is one of the body’s most important organs. It processes food and medicines, helps control energy supplies, makes substances needed for blood clotting and removes waste products, so serious liver damage can affect the entire body.
Damage can build slowly and may not cause symptoms at first. Over time, repeated injury and inflammation can produce scar tissue, and severe widespread scarring is known as cirrhosis.
People with cirrhosis face a higher risk of liver failure and liver cancer. Preventing the earlier stages of liver damage is therefore an important goal, especially as obesity, diabetes, harmful alcohol use and other causes of liver disease remain common.
In the Cedars-Sinai study, people who drank coffee generally had better liver outcomes than those who drank none. The relationship became stronger as coffee consumption increased, although the researchers stressed that this does not mean everyone should start drinking large amounts.
Compared with non-coffee drinkers, participants consuming at least five cups per day had a 32% lower risk of developing cirrhosis. They also had a 47% lower risk of liver cancer and a 42% lower risk of dying from a liver-related cause.
Benefits were not limited to heavy coffee drinkers. The researchers reported favorable associations at one to two cups a day, with the overall benefits appearing strongest at roughly three to four cups.
Instead of relying only on diagnoses, the scientists also looked at what was happening inside the liver. MRI scans showed that greater coffee consumption was associated with lower amounts of liver fat and iron and with fewer signs of inflammation and scarring.
The researchers also examined proteins circulating in the blood. Coffee drinkers tended to have more proteins connected with healthy liver function and fewer proteins associated with inflammation and the formation of scar tissue.
These results give researchers possible biological clues that earlier population studies could not provide. If certain ingredients in coffee influence inflammation or the process that produces liver scarring, that could help explain why coffee drinkers repeatedly show lower rates of liver disease.
Caffeine is an obvious candidate, but the findings suggest the story is more complicated. Both regular and decaffeinated coffee were associated with lower liver risks, indicating that other substances naturally present in coffee may play important roles.
Coffee is a complex mixture containing many plant chemicals. Scientists are now interested in identifying which of these substances might affect liver cells and whether they could eventually reveal new ways to prevent or treat liver disease.
Despite the impressive numbers, the research cannot establish that coffee itself caused the lower disease rates. Participants chose how much coffee they drank, so the researchers could not completely rule out differences in diet, income, exercise, smoking, alcohol use or other behaviors.
This is a key limitation of observational nutrition research. Even when scientists adjust their calculations for many known differences, unknown factors can remain and make one habit appear more protective than it really is.
The study also does not establish an ideal dose of coffee. Five or more cups was associated with lower risks, but that amount could provide too much caffeine for many people, and the researchers specifically said they would not advise people to increase their consumption to that level for liver protection.
Moderate coffee intake may be reasonable for adults who already enjoy it and do not experience unwanted effects. Decaffeinated coffee may also be of interest because it showed similar protective associations without providing the same amount of caffeine.
Coffee should not replace the measures already known to protect liver health. Maintaining a healthy body weight, exercising, limiting alcohol and controlling diabetes, blood pressure and cholesterol remain far more established strategies.
People also respond differently to caffeine. Those with sleep difficulties, severe anxiety, uncontrolled hypertension or certain heart rhythm problems may need to limit it and should seek individual medical advice before changing their intake.
Taken together, the study provides some of the strongest evidence yet that coffee consumption is connected with healthier livers. Its huge sample, long follow-up, MRI findings and blood protein results all point in the same direction, which strengthens the case that the association deserves further study.
Yet the most important unanswered question is whether coffee is truly responsible for the protection. Controlled studies and further laboratory work are needed to identify the active compounds and prove how they affect inflammation and scarring, so for now the findings are encouraging evidence rather than a prescription to drink more coffee.
