
High blood pressure, also known as hypertension, is one of the most common long-term health conditions in the world.
It affects more than 116 million adults in the United States alone and greatly increases the risk of heart attacks, strokes, kidney disease, heart failure, and other serious illnesses.
Because high blood pressure often develops without obvious symptoms, many people do not realise they have it until it has already caused damage.
Although many effective medicines are available, keeping blood pressure under control is still difficult for many people. Some patients do not respond well to treatment, while others experience unwanted side effects. Scientists are therefore searching for new ways to control blood pressure more precisely and safely.
A new study from the University of Virginia has uncovered an important process that helps explain how the body naturally controls blood pressure.
The discovery provides fresh insight into what goes wrong in people with hypertension and may lead to a new generation of treatments that work with fewer side effects than current medicines.
Blood pressure is partly controlled by smooth muscle cells that surround blood vessels. These cells constantly adjust the width of the blood vessels. When the muscle cells tighten, the blood vessels become narrower, making it harder for blood to flow and causing blood pressure to rise. When the muscles relax, the blood vessels widen, allowing blood to flow more easily and lowering blood pressure.
Calcium plays a key role in this process. Tiny changes in calcium levels inside smooth muscle cells tell the muscles when to tighten or relax. Because of this, doctors often prescribe medicines called calcium channel blockers. These drugs reduce the amount of calcium entering the muscle cells, helping blood vessels relax and lowering blood pressure.
However, calcium is also essential for many other functions throughout the body. It helps muscles move, nerves send signals, and many organs work normally. Since calcium channel blockers affect calcium in many different tissues, they can sometimes cause side effects such as dizziness, swollen ankles, headaches, tiredness, or constipation.
The University of Virginia researchers discovered two tiny regions inside smooth muscle cells that they called nanodomains. These microscopic areas act like small control centres, carefully organising calcium signals so blood vessels receive the correct instructions about when to tighten and when to relax.
In healthy blood vessels, the nanodomains keep these signals balanced, allowing blood pressure to adjust naturally as the body’s needs change. In people with hypertension, however, this balance is disrupted. Signals that tell blood vessels to tighten become stronger, while signals that encourage relaxation become weaker. As a result, blood vessels remain narrower than they should, causing blood pressure to stay high.
This finding could eventually change the way high blood pressure is treated. Instead of blocking calcium throughout the entire body, future medicines may be designed to target only these tiny nanodomains. Such an approach could lower blood pressure while reducing unwanted side effects because other calcium-dependent processes would remain largely unaffected.
The research still needs further study before new medicines become available, but it represents an important advance in understanding the causes of hypertension.
By revealing how these tiny control centres regulate blood vessel function, the study offers hope that future treatments could be safer and more effective. For the millions of people living with high blood pressure, this discovery may one day lead to better control of the disease and a lower risk of life-threatening heart attacks and strokes.
