
Photographs of the back of the eye may reveal more than problems with vision.
Researchers at the Mayo Clinic found that damage to tiny blood vessels in the retina could provide clues about a person’s future risk of stroke, dementia and early death.
The retina is a thin layer of light-sensitive tissue at the back of the eye. It contains many very small blood vessels, and because doctors can photograph these vessels directly, the eye provides a rare window into the health of the body’s circulation.
The researchers focused on retinopathy, a condition in which blood vessels in the retina become damaged. Diabetes and high blood pressure are common causes, but other problems affecting blood vessels and circulation can also contribute.
Previous research has connected serious retinopathy with changes seen in the brain. This makes sense because the small blood vessels in the retina share important features with small vessels that supply the brain.
When these vessels become damaged, it may be a sign that similar problems are occurring elsewhere in the body. Poor blood vessel health can reduce the supply of oxygen and nutrients to brain tissue and may contribute to stroke or gradual loss of thinking and memory abilities.
The Mayo Clinic researchers analyzed information from 5,543 adults whose average age was about 56. The information was collected between 2005 and 2008, and the team compared the long-term health of people who had retinopathy with those who did not.
The differences were striking. People with retinopathy were more than twice as likely to have experienced a stroke and were nearly 70% more likely to develop dementia.
They also faced a greater risk of dying during the following decade. That risk increased as retinal damage became more severe, suggesting that the condition of the eye’s blood vessels may reflect broader health problems.
These findings do not mean that retinopathy itself necessarily causes dementia or stroke. Instead, retinal damage may act as a visible warning sign of blood vessel disease that is also affecting the heart, brain or other organs.
This possibility makes retinal photography particularly interesting as a screening tool. Brain imaging with magnetic resonance imaging, or MRI, can provide detailed information, but it is relatively expensive and requires specialized equipment.
Taking a photograph of the retina is much simpler and faster. Modern retinal cameras are widely used in eye care, and some portable systems can work with smartphones or small adapters, potentially making screening available in places where advanced brain imaging is difficult to access.
For someone found to have retinopathy, the most important next step is not to assume that dementia or stroke is inevitable. Instead, the finding may provide another reason to check and manage established risks such as high blood pressure, diabetes, smoking and unhealthy cholesterol levels.
Doctors may also consider a person’s memory and thinking abilities when other warning signs are present. In selected cases, further testing or brain imaging may be appropriate, but retinal photographs cannot currently replace a neurological examination or MRI.
People can also support brain and blood vessel health through everyday habits. Regular physical activity, not smoking, controlling blood pressure and diabetes, getting adequate sleep and eating a balanced diet rich in vegetables, fruits, whole grains, fish and healthy fats are all linked with better cardiovascular and brain health.
Lead author Dr. Michelle P. Lin and colleagues presented the research as evidence that the eyes may provide useful information about health beyond vision. The findings suggest that retinal photography could eventually become a convenient way to identify people who may benefit from closer monitoring and earlier prevention of stroke and dementia.
