Why One Rare Gene Variant Could Protect Against Alzheimer’s Disease

For decades, researchers have wondered why one rare version of the APOE gene seems to protect people from Alzheimer’s disease.

A new study from the Buck Institute for Research on Aging, published in Aging Cell, suggests the answer may lie in the way brain cells defend themselves against everyday damage to their DNA.

Every cell in the body experiences DNA damage over time. Healthy cells constantly repair this damage, but the repair process becomes less effective with aging.

Scientists now believe that failures in DNA repair contribute to memory loss, dementia and other diseases associated with growing older.

The researchers compared the three common forms of the APOE gene using human brain cells grown from stem cells and genetically engineered mice. They discovered that APOE2 consistently helped brain cells stay healthier than APOE3 and APOE4 across different experiments.

Brain cells carrying APOE2 had fewer broken DNA strands and activated stronger repair pathways. They also resisted cellular senescence, an unhealthy condition in which damaged cells stop dividing but continue releasing inflammatory signals. Preventing senescence may help preserve normal brain function during aging.

One surprising finding was that APOE2 protein could partially protect brain cells carrying the high-risk APOE4 gene. After treatment with APOE2 protein, these vulnerable cells showed less DNA damage following stress. This raises hope that future medicines could copy some of APOE2’s natural protective effects.

The researchers also found similar protective changes in the brains of older mice carrying APOE2. The consistency between human cells and animal studies strengthens confidence that the discoveries reflect real biological processes rather than chance laboratory findings.

Although much remains unknown, scientists now have a clearer picture of why APOE2 is linked with longer life and lower Alzheimer’s risk. Future research will explore medicines that improve DNA repair or mimic APOE2 activity in people who inherited APOE4.

Review and analysis: This study is significant because it shifts Alzheimer’s research toward one of the fundamental hallmarks of aging—DNA damage and cellular senescence.

Human clinical studies are still needed, but the findings provide an exciting foundation for developing therapies that protect brain cells before irreversible damage occurs.
