
Statins are some of the most commonly prescribed medicines in the world. Doctors use them mainly to lower LDL cholesterol, often called “bad” cholesterol, and to reduce the risk of heart attacks and strokes.
High levels of LDL cholesterol can contribute to fatty material building up inside arteries. Over time, arteries can become narrower or blocked, making it harder for blood to reach the heart or brain. Large clinical trials have shown that statins can lower the risk of serious heart and blood vessel problems, especially in people already at higher risk.
Despite this evidence, statins remain highly controversial online. A study from Stanford Medicine explored why public discussions about these medicines can look very different from the evidence doctors see in medical research.
Instead of conducting a traditional survey, the researchers studied conversations on Reddit, a large online platform where people discuss health, medicines and personal experiences. The team used artificial intelligence to analyze more than 10,000 posts mentioning statins.
Artificial intelligence allowed the researchers to sort and examine a large amount of written material much faster than people could do manually. They looked for patterns in how users described statins, including whether posts expressed positive, negative or neutral views.
The results showed a strong imbalance. Only about 3% of the posts were clearly positive about statins, while most were negative or neutral.
This does not necessarily mean that most people who take statins have bad experiences. People who experience a problem or feel worried about a medicine may be more likely to post about it. Someone who takes a statin every day without difficulty may have little reason to discuss it online.
This effect can make side effects and negative experiences appear more common than they actually are. Personal stories can also be especially persuasive because readers may find them easier to understand and remember than statistics from large medical studies.
The researchers also found misleading or unsupported health claims in some discussions. Repeated claims can influence readers even when they are not backed by strong scientific evidence.
Distrust of pharmaceutical companies was another common theme. Some Reddit users questioned whether drug companies fully report side effects or accurately describe the benefits of cholesterol-lowering medicines.
Others preferred lifestyle changes instead of medication. Eating a healthy diet, exercising, maintaining a healthy weight and avoiding smoking are important ways to protect the heart. However, lifestyle changes may not lower cholesterol enough for everyone, particularly people with a high inherited or existing risk of heart disease.
The discussions also included alternatives such as ketogenic diets and red yeast rice supplements. Red yeast rice can contain substances that lower cholesterol, but the amount can vary between products, making its effects less predictable than a regulated prescription medicine.
The study highlights a wider challenge in modern healthcare. Social media gives people an important place to share experiences, but the most visible stories do not always represent what happens to the average patient.
For doctors, understanding these conversations could be useful. Knowing which concerns patients encounter online may help healthcare professionals discuss side effects, benefits and uncertainties more clearly rather than simply assuming patients will accept a prescription.
The researchers reported their findings in JAMA Network Open. Their work shows how artificial intelligence can help scientists study large-scale public conversations about health and identify the ideas that may shape decisions about medicines.
People who are considering starting or stopping a statin should discuss the decision with a healthcare professional who understands their individual risk. Online communities can offer useful personal perspectives, but they cannot show whether a particular medicine is the right choice for one person.
Statins have been studied for decades and remain an important tool for preventing heart attacks and strokes.
