
Millions of people have fatty liver disease without realizing it because the condition often causes no symptoms in its early stages.
As the disease worsens, fat inside the liver can trigger inflammation that slowly replaces healthy tissue with scar tissue.
This advanced form, called metabolic dysfunction-associated steatohepatitis or MASH, has become a major cause of liver failure around the world.
When scar tissue continues to build, the liver becomes stiff and cannot work properly. This stage is known as cirrhosis. Although healthy habits remain very important, many patients with advanced disease have had few medical treatments available to stop or reverse the damage.
Researchers from the University of California San Diego School of Medicine have now reported promising results from an international study involving semaglutide. Their research was published on July 15, 2026, in The Lancet Gastroenterology & Hepatology.
Semaglutide is already familiar to many patients because it is used to treat type 2 diabetes and obesity. It helps people lose weight, improves blood sugar control and changes the way the body handles energy. Since obesity and diabetes are closely linked with fatty liver disease, scientists wanted to know whether the medicine could also protect the liver.
Nearly 700 adults with biopsy-confirmed MASH joined the phase 2 trial. Many already had moderate or severe fibrosis, while others had compensated cirrhosis, meaning their liver was scarred but still functioning reasonably well.
The main purpose of the study was to see whether semaglutide combined with an experimental drug called zalfermin could repair liver damage better than either medicine alone. Surprisingly, the combination did not outperform placebo.
Even though the combination disappointed researchers, semaglutide alone delivered encouraging results. People receiving the medicine experienced measurable improvements in liver fibrosis without increasing liver inflammation, suggesting that the drug may help the liver heal while preventing further damage.
Another interesting finding involved monitoring the disease. Blood tests and modern imaging techniques appeared to show treatment progress more clearly than repeated liver biopsies. If confirmed in future studies, doctors may eventually rely less on biopsies, making long-term follow-up much easier for patients.
The scientists emphasised that more work is needed before semaglutide becomes a standard treatment for advanced MASH. Larger trials must determine whether the improvements in scarring translate into fewer cases of liver failure, fewer transplants and longer survival. Other medicines related to zalfermin also deserve further investigation.
This study is important because patients with cirrhosis have often been left out of previous clinical trials. Including these patients provides valuable information about a group that urgently needs better treatments. If future research confirms these findings, semaglutide could become an important option alongside weight management and healthy lifestyle changes.
Review and analysis: The study has several strengths, including its international design, relatively large sample size and use of biopsy-confirmed MASH. However, it was still a phase 2 trial, so the findings should be viewed as encouraging rather than conclusive.
The failure of the combination therapy also reminds us that promising biological ideas do not always produce better patient outcomes. Even so, semaglutide’s positive results justify larger clinical trials and represent meaningful progress for advanced fatty liver disease.
