
A new clinical trial suggests that omega-3 fatty acids combined with low-dose aspirin may help treat severe gum disease almost as well as commonly used antibiotics.
The finding could be important because dentists are looking for ways to reduce unnecessary antibiotic use. The research was published in the Journal of Periodontology.
The study was carried out by researchers from Albert Einstein Israelite Hospital, Guarulhos University, the University of Taubaté and the Ribeirão Preto School of Dentistry at the University of São Paulo in Brazil, together with Harvard University in the United States.
The researchers tested whether helping the body control inflammation could improve healing after standard dental treatment. Their results offer a possible new approach for some patients with advanced gum disease.
Periodontitis is a serious, long-lasting form of gum disease. It develops when bacteria collect around and below the gum line and the body’s response to the infection causes ongoing inflammation. Over time, this process can damage the gums and bone that keep teeth firmly in place.
As the disease becomes worse, deep spaces can form between the teeth and gums. These areas are difficult to clean and can hold harmful bacteria. Without effective treatment, the supporting bone may gradually disappear and teeth can eventually become loose or fall out.
The main treatment is a deep cleaning below the gum line that removes bacteria and hardened deposits from tooth roots. For severe cases, dentists may also prescribe antibiotics such as amoxicillin and metronidazole. Antibiotics can help, but using them when they are not necessary contributes to antibiotic resistance, which makes bacterial infections harder to treat.
The new trial included 109 people with advanced periodontitis and followed them for one year. Everyone received standard deep cleaning, but they were randomly placed into groups that received different additional treatments. One group received inactive capsules, while another took amoxicillin and metronidazole three times daily for 14 days.
A third group took three grams of omega-3 each day together with low-dose aspirin for six months. A fourth group received both the two-week antibiotic treatment and the six-month omega-3 and aspirin treatment. Researchers examined the participants after three, six and 12 months to measure how well their gums had healed.
After one year, about 58% of people who received antibiotics met the researchers’ treatment goal, meaning they had no more than four deep gum pockets left. The omega-3 and low-dose aspirin group produced an almost identical result, with 57.7% reaching the goal. Only 23.1% of those who received deep cleaning plus inactive capsules reached the same level of improvement.
Adding antibiotics to omega-3 and aspirin did not appear to provide extra benefit. This was unexpected because the researchers had thought that combining the treatments might produce the strongest result. Instead, omega-3 and low-dose aspirin alone performed similarly to the antibiotic treatment.
Omega-3 fats, which are naturally found in foods such as oily fish, can help the body manage inflammation. Scientists believe the body can turn omega-3 fats into substances that help bring inflammation to an end and support healing. Low-dose aspirin may encourage the production of some of these substances, which could help damaged gum tissue recover after deep cleaning.
Interestingly, the improvements remained after participants stopped taking omega-3 and aspirin at six months. When researchers checked them again six months later, the benefits were still similar, although it is not yet clear why the effect lasted. The team is also studying changes in bacteria inside the gum pockets to better understand what happened.
The findings do not mean people with gum disease should begin taking omega-3 supplements and aspirin without medical advice. Aspirin can increase bleeding and may not be safe for people with certain health conditions or those taking some medicines.
The trial was also relatively small and included patients without major additional diseases, so larger studies involving more diverse groups are needed.
If future research confirms the findings, omega-3 and low-dose aspirin could become an alternative for selected patients and may help reduce the need for antibiotics in periodontal care. For now, standard dental treatment remains essential, and any additional medicine or supplement should be discussed with a dentist or doctor.
