Brain Stimulation May Ease Hidden Parkinson’s Symptoms

Parkinson’s disease is best known for movement problems such as shaking, stiff muscles, and slow movement.

But many people with the disease also live with depression, anxiety, and a loss of motivation.

A new study from Korea University Ansan Hospital suggests that a gentle form of brain stimulation may help with several of these problems at the same time.

The research focused on transcranial direct current stimulation, commonly shortened to tDCS.

The treatment sends a very weak electrical current through electrodes placed on the scalp to influence activity in selected parts of the brain. It does not require surgery or anesthesia, and treatment sessions are generally associated with little discomfort.

Researchers have been studying tDCS for a range of brain and mental health conditions because it may change how groups of brain cells communicate. In the new study, scientists wanted to know whether it could improve depression and apathy in people with Parkinson’s disease.

They were also interested in whether any improvement would make a noticeable difference in everyday life.

That second question is important because Parkinson’s is much more than a movement disorder. The disease develops as brain cells involved in producing the chemical messenger dopamine become damaged and die. This affects movement, but changes in the brain can also influence mood, thinking, sleep, motivation, and other parts of daily life.

Apathy can be especially difficult for patients and families. A person with apathy may have less interest in activities, find it difficult to begin tasks, or appear emotionally less engaged even when they are not necessarily feeling sad. This is different from depression, although the two conditions can occur together.

The study was led by Professor Ho Kyung Yoon of the Department of Psychiatry and Professor Do Young Kwon of the Department of Neurology at Korea University Ansan Hospital. The researchers enrolled 20 people with Parkinson’s disease who were experiencing depressive symptoms. Participants received 10 tDCS sessions, with treatment given three times each week to brain areas involved in emotional control.

The team added an unusual feature to the study by asking participants to wear smart bands. These devices recorded information about daily movement, including step counts, as well as sleep patterns. This allowed the researchers to look beyond questionnaires and clinic examinations and see whether patients actually became more active in their normal surroundings.

After the treatment period, several measures improved. Depressive symptoms fell by about 54%, while anxiety symptoms decreased by around 38%. Apathy improved by approximately 25%, and measures of Parkinson’s movement problems, including stiffness and slow movement, improved by about 35%.

The wearable devices also detected a meaningful increase in spontaneous daytime activity after treatment. In other words, participants were not simply reporting that they felt better during clinic visits. They appeared to be moving more during ordinary daily life as well.

One of the most interesting findings involved motivation. The researchers found that improvement in apathy was the strongest predictor of increased daily physical activity. The connection between reduced apathy and greater activity was stronger than the connection between improved depression and activity, and it remained even when age and the length of time a person had Parkinson’s were considered.

This finding may matter because increasing physical activity can be difficult when a person lacks the motivation to start. If a treatment helps people become more interested in and willing to engage with everyday activities, its benefits could extend beyond scores on a medical test. Wearable devices may give doctors a practical way to detect such changes.

The study was published in the journal Acta Neuropsychiatrica. The authors suggest that combining noninvasive brain stimulation with wearable monitoring could eventually support more personalized Parkinson’s care by showing doctors how a patient responds outside the clinic.

The results are encouraging, but they should be viewed as early evidence rather than proof that tDCS should become a standard Parkinson’s treatment. Only 20 patients took part, which is a very small group, and larger controlled studies are needed to determine how much of the improvement was caused by the stimulation itself. Researchers will also need to learn how long the benefits last and which patients are most likely to respond.

Still, the study has an important strength: it connects clinical measurements with real-world behavior. Parkinson’s treatments are most valuable when they help people function better in daily life, not simply when they improve a score during an appointment.

Using wearable devices alongside carefully designed clinical trials could provide a fuller picture of whether new treatments genuinely help patients live more active and engaged lives.
