Study Finds a Big Cause of Severe Inflammation

Scientists have identified a rare genetic disease that can cause serious inflammation of blood vessels and scarring in the liver from very early in life.

The condition, called Lyn kinase-associated vasculopathy and liver fibrosis, or LAVLI, was linked to changes in a gene known as LYN.

The discovery gives researchers a new window into how the immune system controls inflammation. It may also help scientists explore more targeted treatments for rare inflammatory diseases involving blood vessels and the liver.

LAVLI belongs to a broad group of disorders in which the body’s natural defenses produce too much inflammation. These conditions can develop when parts of the immune system become active at the wrong time or remain active for too long.

This differs from classic autoimmune disease, in which the immune system mistakenly targets the body’s own cells using an immune response directed against them. Autoinflammatory disorders are more closely connected with problems in the body’s rapid, first-line immune defenses.

The researchers began investigating the condition after doctors encountered a child with an unusual combination of symptoms. Genetic testing found a change in the LYN gene, and two additional children were later identified with different changes affecting the same gene.

All three children developed serious health problems very early in life. One major feature was inflammation affecting small blood vessels in the skin, with large numbers of immune cells called neutrophils gathering around the vessels.

Neutrophils normally help the body fight infections. But when they become excessively active or collect in the wrong place, they can contribute to inflammation and damage healthy tissue.

Two of the three children also developed liver fibrosis during their first year. Fibrosis happens when repeated injury causes scar tissue to build up, and severe or continuing liver fibrosis can eventually interfere with the organ’s ability to work normally.

The researchers traced these problems to abnormal activity of a protein called Lyn kinase, which is produced using instructions from the LYN gene. Lyn kinase helps immune cells receive and control signals that tell them when and how to respond.

In the children with LAVLI, the genetic changes made Lyn kinase unusually active. Instead of being carefully controlled, the protein continued sending signals that affected the movement and behavior of immune cells.

This excessive signaling appeared to drive ongoing inflammation. The researchers believe it contributed to both the blood vessel damage and the liver scarring seen in the children.

The finding is important because rare diseases can sometimes reveal basic biological processes that are relevant to much more common illnesses. Understanding how excessive Lyn kinase activity produces inflammation could eventually provide clues about other conditions involving inflamed blood vessels or tissue scarring.

It also raises the possibility of treatments designed to reduce abnormal Lyn kinase signaling. However, LAVLI has been identified in only a very small number of patients, so much more research is needed before scientists can know which treatments are safe and effective.

The discovery also demonstrates the growing importance of genetic testing for children with unexplained and unusual combinations of symptoms. Finding a disease-causing genetic change can give doctors a clearer diagnosis and may help guide future treatment as researchers learn more about the condition.

The research was led by Dr. Adriana A. de Jesus and colleagues at the National Institute of Allergy and Infectious Diseases. The findings were published in Nature Communications.

For families affected by extremely rare diseases, reaching a diagnosis can sometimes take years. Discoveries such as LAVLI show how genetic research can connect seemingly unrelated symptoms and reveal the biological cause behind them.

Researchers will now need to identify additional patients and study the disease over longer periods. Doing so could reveal how LAVLI develops, how widely its symptoms vary and whether medicines that control the abnormal immune signaling can prevent serious organ damage.
