
Cavities in young children are often treated only after they become painful, but a new study suggests there may be a much simpler solution.
Researchers led by the University of Michigan have shown that a quick paint-on liquid can stop tooth decay without drills, needles, or sedation. The findings were published in JAMA Pediatrics.
Untreated cavities are more than a minor inconvenience. They can lead to severe pain, infections, trouble eating, sleep problems, and even emergency hospital visits. In many cases, very young children need dental treatment under general anesthesia because traditional procedures are too difficult for them.
The treatment tested in the study is called silver diamine fluoride, or SDF. Instead of removing decayed tooth material, dentists simply brush a small amount of the liquid directly onto the cavity. The process is fast, painless for most children, and can be repeated every few months if needed.
The Phase III study included 830 children younger than six years from several U.S. states. Scientists wanted to determine whether SDF could reliably stop cavities in a large American population and provide the evidence needed for FDA approval as a cavity treatment.
The results were encouraging. More than half of the treated baby teeth had their decay successfully arrested after applications every six months. This means the cavities stopped progressing, reducing the chance of pain, infection, or emergency treatment.
The study is especially important because SDF has already been used successfully around the world for many years. Until now, however, there had not been enough large, carefully designed U.S. studies to satisfy regulatory requirements for broader approval.
Researchers say the treatment could make dental care much more accessible for children who rarely see a dentist, as well as older adults, people with disabilities, and individuals with extreme dental anxiety. Pediatricians may even be able to identify children who could benefit while waiting for dental care.
The only major drawback is that the decayed part of the tooth becomes permanently dark after treatment. For many families, however, preventing pain, infection, and surgery may outweigh the cosmetic concern, especially because baby teeth eventually fall out naturally.
This research is one of the most important studies of silver diamine fluoride performed in the United States because it included a large number of young children and used a rigorous clinical trial design.
The results strongly support broader use of SDF and may lead to FDA approval, improved insurance coverage, and earlier treatment of childhood cavities. While it is not a replacement for good oral hygiene or regular dental care, it could become a valuable public health tool for preventing serious dental disease.
