Taking Stairs Could Lower Early Death Risk

A healthier heart may sometimes be only a staircase away.

A large analysis led by researchers at the University of East Anglia suggests that people who make stair climbing part of everyday life may have a lower chance of heart disease and premature death.

The idea matters because lack of exercise is a major public health problem. Modern life allows many people to spend most of the day sitting, while elevators, cars and other conveniences remove small amounts of movement that were once part of daily routines.

The human body, however, benefits from regular physical effort. When people move more, the heart and lungs are challenged, muscles use more energy and the body becomes better at handling blood sugar, blood pressure and fats in the blood.

Health guidelines commonly encourage adults to get regular moderate or vigorous physical activity. But a long gym session is not the only way to be active, and researchers are increasingly studying whether short bursts of movement spread through the day can also improve health.

Stair climbing is an interesting example because it quickly raises the heart rate. Lifting the body upward against gravity takes more effort than ordinary walking, so even a few flights can provide a brief period of fairly vigorous activity.

Researchers from the University of East Anglia and Norfolk and Norwich University Hospital wanted to know whether this everyday habit was connected with major long-term health outcomes. They therefore brought together evidence from previous studies rather than relying on a single group of participants.

The team searched nearly 1,900 scientific studies and selected nine that met the requirements for their final analysis. Together, these studies included more than 480,000 adults, with ages ranging from 35 to 84.

About 53% of the participants were women, and the groups included people with and without existing cardiovascular disease. The median follow-up was about 14 years, allowing researchers to compare stair-climbing habits with later illness and death.

The difference between stair climbers and non-climbers was substantial. Regular stair climbing was associated with a 39% lower risk of dying from cardiovascular disease during follow-up.

The researchers also found a 24% lower risk of death from any cause. People who climbed stairs were less likely to develop several major cardiovascular conditions, including heart attacks, strokes and heart failure.

These results fit with a wider body of research showing that small amounts of vigorous movement may matter. Short stair-climbing sessions can challenge the cardiovascular system and, when repeated, may improve the body’s ability to deliver oxygen during activity.

There may also be benefits for other heart risk factors. Earlier research has reported improvements in fitness and cholesterol after repeated stair climbing, although the exact results depend on the people studied and the exercise program used.

How much climbing is necessary remains unclear. One large study cited by the researchers suggested that around six flights each day could provide strong benefits, but there is not yet enough evidence to turn that number into a recommendation for everyone.

That is one of the study’s important limitations. The included research did not measure stair climbing in exactly the same way, and people may not always accurately remember or report how often they use stairs.

There is another issue that makes careful interpretation necessary. People who regularly choose stairs may already be healthier, younger in biological terms, more active or less limited by illness than people who avoid them, and those differences could partly explain their lower risks.

Because of this, the analysis shows an association rather than proving that stairs directly prevent heart attacks or extend life. A randomized trial assigning people to different amounts of stair climbing would provide stronger evidence, although such a long-term study would be difficult to conduct.

The researchers also stress that stairs are not the right exercise for every person. Someone with balance difficulties, painful joints, severe mobility problems or health conditions that make vigorous exertion unsafe may benefit more from another form of movement.

The study, “Evaluating the Impact of Stair Climbing on Cardiovascular Risk Reduction: A Systematic Review and Meta-analysis,” was published in the American Journal of Cardiovascular Drugs. The research adds to evidence that ordinary activities can contribute to cardiovascular fitness when they are performed regularly.

The strongest feature of the study is its large combined sample and long follow-up, which allowed the researchers to examine serious outcomes rather than only short-term changes in fitness. Its main weakness is that the underlying evidence was largely observational and therefore cannot completely separate the effects of stair climbing from the characteristics of people who choose to climb.

Overall, the findings support a simple idea rather than a miracle exercise. For people who can climb stairs safely, choosing them more often is an easy way to add movement to the day, but it should be viewed as one part of a broader healthy lifestyle that also includes regular activity, a balanced diet, not smoking and control of blood pressure, cholesterol and diabetes.
