
Pancreatic cancer remains one of the hardest cancers to treat successfully. Even when surgeons remove the entire visible tumor, many patients do not survive for long because the disease often returns.
Scientists are searching for simple clues that may help predict which patients are likely to have better outcomes after surgery.
A research team from Hiroshima University has found an unexpected possibility: the number of natural teeth a patient still has. Their study, published in the Journal of Hepato-Biliary-Pancreatic Sciences, suggests that oral health could be an important indicator of long-term survival following surgery for pancreatic ductal adenocarcinoma.
Researchers have long known that poor oral health is linked with heart disease, diabetes, and several cancers. Gum disease can cause long-lasting inflammation and allow harmful bacteria to enter the bloodstream. Some of these bacteria have even been connected with pancreatic cancer in earlier laboratory studies.
To investigate whether oral health influences survival, the researchers followed 339 patients who underwent radical pancreatic surgery between 2014 and 2022. Every patient received a dental examination before the operation. The team counted natural teeth and examined how well the upper and lower back teeth fitted together.
The results were striking. Patients with at least 21 natural teeth lived for a median of just over 60 months after surgery, while those with 20 or fewer teeth lived for about 39 months. This gap of nearly two years remained even after the researchers adjusted for many well-known factors that affect pancreatic cancer survival.
One surprising discovery was that chewing function itself did not explain the difference. Instead, the researchers believe the number of remaining teeth may reflect a person’s overall health accumulated over many decades. Good oral health may indicate lower chronic inflammation, better nutrition, healthier habits, and greater physical resilience.
The study supports the growing idea that the mouth can provide valuable information about general health. Rather than viewing dental care separately from medical care, future healthcare may treat oral health as an important part of cancer management and recovery.
However, the researchers emphasised that this was an observational study conducted at a single hospital in Japan. The findings do not prove that improving dental health alone will increase survival. Larger international studies are needed to determine whether the same relationship exists in different countries and healthcare systems.
The team also plans to investigate how oral bacteria and chronic inflammation might influence pancreatic cancer growth. Understanding these biological processes could eventually lead to new ways of reducing cancer progression or improving recovery after surgery.
Overall, the research opens an important new direction in pancreatic cancer care. It suggests that successful treatment may depend not only on removing the tumor but also on the patient’s overall health, including oral health built over a lifetime.
The study is thought-provoking and clinically important, although stronger evidence from larger studies will be needed before dental assessments become a routine part of pancreatic cancer treatment.
