A Simple Paint-On Treatment Could End Painful Cavities

Tooth decay is the most common long-term disease affecting children, yet many young children still do not receive treatment before their cavities become serious.

Every year, thousands of children in the United States end up in emergency departments because of severe tooth pain and infection.

A new study led by the University of Michigan suggests that a simple liquid treatment may help prevent many of these painful experiences. The research was published in JAMA Pediatrics.

Many young children are frightened of dental drills and injections, and some require treatment under general anesthesia because they are too young to sit through traditional dental procedures. This creates extra risks, costs, and long waiting times for families.

The treatment studied is called silver diamine fluoride, or SDF. Dentists simply paint the clear liquid onto a cavity with a small sponge. The procedure takes only a few seconds, requires no drilling, no injections, and usually causes little discomfort.

SDF has been used successfully in many countries for decades and has been available in the United States since 2014 to treat tooth sensitivity. Although dentists have also used it to stop cavities, it has remained an off-label treatment because large U.S. clinical trials had not been completed.

To provide stronger evidence, researchers carried out a Phase III clinical trial involving 830 children younger than six years of age. Children were recruited from dental clinics, pediatric medical practices, and Head Start programs in Michigan, New York, and Iowa.

The researchers found that 38% silver diamine fluoride successfully stopped tooth decay in more than half of the affected baby teeth when it was reapplied every six months. This means many cavities stopped getting worse without the need for fillings or more invasive treatment.

Professor Margherita Fontana, who led the study, said the treatment proved to be both effective and safe, even for children as young as one year old. This is important because early childhood tooth decay can cause pain, infection, difficulty eating, poor sleep, missed school, and repeated medical visits.

The researchers believe SDF could also help older adults, people with disabilities, and patients with severe dental anxiety who struggle to receive conventional dental care. For many people, it could stop decay while they wait for more permanent treatment or even avoid it altogether.

The main disadvantage is cosmetic. The decayed part of the tooth turns black permanently after treatment. However, the researchers believe many families may accept this trade-off if it means avoiding pain, infection, hospital visits, or surgery.

This large U.S. clinical trial provides the strongest evidence so far that silver diamine fluoride is a safe and effective way to stop cavities in young children. The findings could support FDA approval and encourage wider use by dentists, pediatricians, and insurers.

Although the discoloration remains a limitation, the treatment offers an affordable and practical solution that could greatly improve children’s dental health and reduce unnecessary surgeries.
