
For many people with knee osteoarthritis, everyday activities such as climbing stairs, standing from a chair or walking around the shops become painful.
As the condition progresses, chronic pain can reduce independence, disturb sleep and lower quality of life. Finding treatments that are both effective and easy to continue is therefore an important goal for doctors and researchers.
Scientists at the University of Nottingham recently investigated whether supporting the gut microbiome could help. Their study, published in Nutrients, focused on inulin, a prebiotic fibre that naturally occurs in several vegetables. Rather than acting directly on the knee joint, the fibre works by feeding beneficial bacteria living in the digestive system.
Over the past decade, scientists have learned that gut bacteria do much more than help digest food. They also produce chemicals that communicate with the immune system, the brain and muscles. This growing field of research has led scientists to wonder whether changing the gut microbiome could reduce chronic pain in diseases such as osteoarthritis.
The INSPIRE clinical trial enrolled 117 adults with knee osteoarthritis. Participants were divided into four treatment groups, receiving inulin, digital physiotherapy, both treatments together or a placebo. The trial lasted six weeks and compared changes in pain, physical function and several biological markers.
The researchers found that both inulin and physiotherapy reduced knee pain. However, people taking inulin experienced additional benefits. Their grip strength improved, and they became less sensitive to painful stimulation, suggesting changes in the way their nervous system processed pain.
Blood and gut measurements helped explain why these improvements may have occurred. Participants taking inulin produced more butyrate, a beneficial substance made by gut bacteria, and had higher levels of GLP-1, a hormone involved in metabolism, muscle health and pain regulation. These biological changes matched the improvements seen in physical testing.
One practical finding stood out. Very few participants stopped taking the fibre supplement, whereas many more withdrew from the physiotherapy program. This does not mean exercise is unimportant, but it suggests that a simple daily supplement may be easier for some people to fit into their routine.
The researchers believe the study reveals a possible connection between gut health, muscles and pain. If future research confirms this gut-muscle-pain pathway, it could open the door to new treatments not only for osteoarthritis but also for other conditions linked to ageing and chronic inflammation.
People should not assume that fibre supplements alone can replace medical care. Regular exercise, maintaining a healthy body weight, eating a balanced diet and following healthcare advice remain the foundation of osteoarthritis management. Inulin may eventually become an additional option rather than a replacement for existing therapies.
This research is exciting because it combines nutrition, gut health and arthritis in a scientifically rigorous randomised controlled trial. Nevertheless, the trial was small and short, so its results should be viewed as preliminary.
Larger studies lasting many months will be needed to determine whether the improvements continue over time and whether different groups of patients benefit equally. If confirmed, this approach could provide a simple, low-cost addition to current osteoarthritis care.
