
A new artificial intelligence system could give doctors an early warning when stroke patients are about to develop one of the deadliest complications of the disease.
Scientists at Washington University School of Medicine have created software that predicts dangerous brain swelling before patients become critically ill.
The technology recently received FDA Breakthrough Device designation, an important step toward clinical approval.
Brain swelling after a major stroke is called malignant cerebral edema. As swelling increases, pressure builds inside the skull because there is little room for the brain to expand.
Without rapid treatment, this pressure can permanently damage the brain and is often fatal.
Surgeons can remove part of the skull to relieve pressure, but timing is essential. Waiting until symptoms become obvious may reduce the benefit of surgery, making early prediction extremely valuable.
The new software, BeaconPredict, was designed to solve this problem. Instead of relying only on a doctor’s interpretation, it combines brain imaging with blood pressure, age, neurological examination findings, and other clinical information. Artificial intelligence then estimates the patient’s likelihood of developing severe brain swelling.
The researchers evaluated the system using data from 598 stroke patients treated at three medical centers. According to the study, BeaconPredict identified patients who later required surgery with approximately 99% accuracy, clearly outperforming conventional assessment methods.
The software was also much better at avoiding unnecessary surgery recommendations. This is important because brain surgery carries risks, so doctors need confidence that only the patients most likely to benefit are selected.
The FDA Breakthrough Device program supports technologies that may significantly improve treatment for serious illnesses. While this designation does not mean the device is fully approved, it allows developers to work more closely with regulators during the review process.
The researchers hope the software will eventually become part of routine stroke care. Because it analyzes information already collected in hospitals, it could fit naturally into existing clinical workflows without requiring major changes.
Like all AI systems, BeaconPredict will need continued testing in larger and more varied healthcare settings before widespread adoption. Researchers must also show that using the software changes treatment decisions and improves survival compared with current care.
