
An Australian study has found that swimming can be an effective way to reduce pain and disability caused by chronic low back pain.
The research, led by Macquarie University and published in the British Journal of Sports Medicine, is the first clinical trial to show that a structured swimming program can produce meaningful improvements for people living with long-lasting back pain.
Chronic low back pain is one of the leading causes of disability around the world.
It affects millions of people and can make it difficult to work, exercise, sleep, or enjoy daily activities. Doctors usually recommend staying active because long periods of rest can make the problem worse.
Swimming has often been suggested as a good form of exercise because the water supports the body and reduces pressure on the spine.
Even so, there has been little scientific evidence showing whether swimming truly helps people with chronic back pain. The new study was designed to answer that question.
The trial included 76 adults between 26 and 74 years of age who had experienced low back pain for at least 12 weeks. All participants had some level of disability caused by their pain. They were randomly assigned to either a swimming program with education or an education-only group.
The swimming group completed an individualized eight-week program designed by a physiotherapist. Participants were encouraged to gradually build up to three swimming sessions lasting 30 to 45 minutes each week. They also received telehealth coaching and learned key ideas about back pain, including that movement is usually safe and flare-ups do not always mean new injury.
The comparison group received the same educational advice from a physiotherapist but continued with their usual activities and treatments. This allowed the researchers to determine whether swimming provided extra benefits beyond education alone.
After eight weeks, the swimming group showed much greater improvements. Disability scores fell by about 50% compared with the start of the study, and participants experienced less pain, better physical function, greater confidence in managing their pain, and less fear of movement.
The benefits remained noticeable six months and one year later, although the difference between the two groups became smaller over time. Many participants said swimming gave them the confidence to stay active, with some continuing to swim and others moving on to different forms of exercise.
The researchers said the improvements compared well with results reported for other recommended exercises such as Pilates. They believe swimming is especially helpful because the buoyancy of water makes movement easier while still improving fitness, muscle strength, and flexibility.
This study provides strong evidence supporting swimming as a treatment option for chronic low back pain. However, participants followed a supervised program with physiotherapy guidance, so the results may not apply to everyone who simply starts swimming on their own.
Future research involving larger groups could help identify which patients benefit the most and how to maintain the improvements over the long term.
