Study finds safer treatment of high blood pressure

A study from Columbia University has raised important questions about how doctors treat high blood pressure.

The research suggests that one of the most commonly used medicines, called ACE inhibitors, may not be the best first choice for many patients. Instead, another type of drug, known as thiazide diuretics, may be safer and more effective.

High blood pressure, also called hypertension, is a serious health condition that affects millions of people around the world. It happens when the force of blood pushing against the walls of the arteries is too high.

Over time, this can damage the heart and blood vessels, leading to heart attacks, strokes, and other serious problems. Because of these risks, many people need medication to control their blood pressure.

Doctors have several types of medicines to choose from when treating high blood pressure. These include thiazide diuretics, ACE inhibitors, angiotensin receptor blockers, calcium channel blockers, and others.

However, deciding which one to start with is not always easy. In many cases, treatment guidelines are based on expert opinions rather than strong direct comparisons between drugs.

To better understand which treatment works best, researchers analyzed data from nearly 5 million patients who were starting blood pressure treatment for the first time. They used health records and insurance data to compare outcomes between different medications.

The results showed that almost half of the patients, about 48%, were given ACE inhibitors as their first treatment. In contrast, only 17% started with thiazide diuretics. This shows that ACE inhibitors are currently the more popular choice among doctors.

However, the study found that patients who started with thiazide diuretics had better results. They had a 15% lower risk of major heart-related problems, such as heart attacks, strokes, and heart failure. This is an important finding because it suggests that choosing a different starting treatment could prevent many serious health events.

The researchers also looked at side effects. They found that ACE inhibitors were linked to more problems. Patients taking these drugs experienced higher rates of 19 different side effects. These included issues such as a long-lasting cough and higher levels of potassium in the blood.

On the other hand, patients who took thiazide diuretics reported fewer side effects overall. This means that not only were thiazides more effective in reducing serious heart problems, but they were also easier for patients to tolerate.

The research team, led by George Hripcsak, estimated that if all the patients in the study had started with thiazide diuretics instead of ACE inhibitors, about 3,100 major heart-related events could have been avoided. This shows how much impact a simple change in treatment choice could have on public health.

Thiazide diuretics work by helping the body remove extra salt and water through urine. This reduces the amount of fluid in the blood vessels, which lowers blood pressure.

ACE inhibitors, in contrast, work by relaxing blood vessels through a different process. While both types of drugs can lower blood pressure, this study suggests that thiazides may provide better overall protection.

The study was published in the medical journal The Lancet. It provides strong evidence that doctors may need to rethink how they choose the first medication for treating high blood pressure.

For patients, this research highlights the importance of discussing treatment options with a doctor. Not all medications work the same way for everyone, and the best choice may depend on a person’s health, medical history, and risk factors.

In the end, this study shows that using data from large numbers of patients can help improve medical decisions. By choosing treatments that are both effective and have fewer side effects, doctors can provide better care and help prevent serious heart problems.
