Common constipation drug may help protect kidneys

Chronic kidney disease, often called CKD, is a serious and long-lasting condition that affects millions of people around the world. The kidneys play a very important role in the body.

They filter waste and extra fluid from the blood, helping to keep the body clean and balanced. When the kidneys slowly lose this ability, waste can build up in the body and cause many health problems.

For people with advanced CKD, life can become very difficult. Many need regular dialysis, a treatment that removes waste from the blood when the kidneys can no longer do their job.

Dialysis can help people survive, but it does not cure the disease, and it can greatly affect daily life. At the moment, there are no approved drugs that can truly restore kidney function or stop the disease completely.

However, new research from Tohoku University in Japan has brought fresh hope. A team led by Professor Takaaki Abe has found that a drug normally used to treat constipation may help protect the kidneys and slow down the progression of CKD. The findings were published in the scientific journal Science Advances.

The drug is called lubiprostone. It has been used for many years to treat constipation by helping the body move waste through the digestive system more easily.

At first, it might seem unrelated to kidney health, but the researchers noticed something interesting. Many people with chronic kidney disease also suffer from constipation. This led them to wonder if there could be a connection between gut health and kidney function.

The human body is complex, and different systems are closely linked. The researchers found that when a person is constipated, the balance of bacteria in the gut can change.

This imbalance, often called gut dysbiosis, can produce harmful substances that may enter the bloodstream and damage the kidneys over time. Based on this idea, the team asked a simple but important question: could improving gut health help protect the kidneys?

To explore this idea, they carried out a clinical trial with 150 patients who had moderate chronic kidney disease. The study took place in nine hospitals across Japan. Some patients were given lubiprostone, while others continued their usual treatment. The researchers then monitored how the patients’ kidney function changed over time.

Kidney function was measured using a common test called estimated glomerular filtration rate, or eGFR. This test shows how well the kidneys are filtering blood. A lower eGFR means worse kidney function.

The results of the study were encouraging. Patients who took lubiprostone showed a slower decline in their eGFR compared to those who did not take the drug. In simple terms, their kidneys stayed healthier for longer. The effect was even stronger in patients who took higher doses of the medication.

The researchers also wanted to understand why this drug had such an effect. They discovered that lubiprostone increases the level of a natural substance in the body called spermidine.

Spermidine supports healthy gut bacteria and helps improve the function of mitochondria. Mitochondria are small structures inside cells that produce energy. When mitochondria work well, cells are healthier and more able to resist damage.

By improving gut health and boosting mitochondrial function, lubiprostone appears to create a protective effect for the kidneys. This is a new way of thinking about kidney disease. Instead of focusing only on the kidneys themselves, this approach looks at the whole body, especially the connection between the gut and other organs.

The research team plans to continue their work by testing the drug in larger groups of patients. They also hope to find ways to predict which patients will benefit the most, so treatments can be more personalized in the future.

This discovery could change how doctors treat chronic kidney disease. It suggests that simple treatments, such as managing constipation and improving gut health, may have powerful effects on serious diseases. It also opens the door to studying similar treatments for other conditions related to energy problems in cells.

While more research is needed, this study offers real hope. It shows that new ideas can come from looking at old treatments in a different way. For people living with CKD, even small improvements can make a big difference in quality of life.
