New drug shows promise in protecting kidneys in type 2 diabetes

People with type 2 diabetes often face serious health challenges, especially if they also have kidney disease. These individuals are at high risk of kidney failure, heart attacks, strokes, and even death. For years, the main treatment to protect their kidneys has been a therapy called renin-angiotensin-aldosterone system (RAAS) blockade.

While this therapy has been helpful, it has its limits. Now, researchers at Stanford University have found a new way to help these patients, using a drug called canagliflozin.

The study, which involved 4,401 participants from 34 different countries, aimed to see if canagliflozin could do a better job of protecting kidneys than the current standard therapy. All participants were already receiving the best available care for kidney disease, which included RAAS therapy.

They were then randomly divided into two groups: half received canagliflozin, and the other half received a placebo—a harmless pill with no medical effect.

Canagliflozin works in a unique way. It belongs to a class of drugs called SGLT2 inhibitors. Normally, when blood passes through the kidneys, a lot of sugar is reabsorbed back into the bloodstream.

Canagliflozin blocks this process, causing the kidneys to remove more sugar from the body through urine. This helps lower blood sugar levels, which is important for managing diabetes. But this study shows that it also has a protective effect on the kidneys and the heart.

In fact, canagliflozin is not new to the medical world. It has already been approved by the U.S. Food and Drug Administration (FDA) for lowering blood sugar levels in people with type 2 diabetes.

It is also approved for reducing the risk of heart problems in people with both type 2 diabetes and existing heart disease. This new research adds to its list of benefits by showing it can also protect against kidney failure.

This discovery is a big step forward for people with type 2 diabetes and kidney disease. Until now, RAAS blockade therapy was the only approved method to slow down kidney damage.
