
Many people believe their blood pressure stays about the same from day to day. However, new research suggests that blood pressure can change much more than most people realise, even in those who take medication every day.
These natural ups and downs can make it difficult for both patients and doctors to know whether treatment is working as well as expected.
High blood pressure, also called hypertension, is one of the most common long-term health conditions in the world. It places extra strain on the heart and damages blood vessels over time, increasing the risk of heart attack, stroke, kidney disease, and memory problems.
Although effective medicines are available, many people still struggle to keep their blood pressure within a healthy range.
A research team led by Yuan Lu from Yale School of Medicine wanted to understand how much blood pressure changes between medical appointments. Their findings were published in the journal Circulation: Cardiovascular Quality and Outcomes. The study is one of the largest ever carried out on this topic.
The researchers analysed more than 7.7 million blood pressure measurements collected from over 537,000 adults. They found that blood pressure often changed by as much as 12 millimetres of mercury (mmHg) between clinic visits. Such large changes were common, even among people who were already taking medicine to lower their blood pressure.
The study also showed that blood pressure medicines did not always appear to produce a clear improvement. In about 37 percent of cases, there was little or no obvious reduction in blood pressure between appointments.
Because blood pressure naturally rises and falls, it can be difficult to tell whether the medicine is truly helping or whether the change is simply part of normal variation.
This finding has important consequences for medical care. If a doctor relies on only one reading taken during a clinic visit, the result may not accurately reflect a person’s usual blood pressure. A treatment might appear to be working when it is not, or a medicine might be changed unnecessarily because of a temporary increase in blood pressure.
For this reason, health experts, including the American Heart Association, recommend checking blood pressure regularly at home. Home monitoring provides many readings over time, giving doctors a much clearer picture of how blood pressure behaves during everyday life instead of relying on occasional clinic visits.
Experts also point out that blood pressure measured in a doctor’s office may sometimes be higher than normal. Feeling nervous during an appointment, often called the ‘white coat effect,’ can temporarily raise blood pressure. Using a cuff that does not fit correctly may also affect the accuracy of the reading.
People who monitor their blood pressure at home should use a reliable upper-arm monitor whenever possible. Before taking a reading, it is best to avoid smoking, drinking caffeine, or exercising for at least 30 minutes.
Sitting quietly with both feet on the floor, the back supported, and the arm resting at heart level can also improve accuracy. For those who cannot afford a home monitor, many pharmacies and community health centres provide free blood pressure machines.
The study reminds us that blood pressure is not a fixed number but something that naturally changes throughout the day and over longer periods. Tracking it regularly rather than relying on a single measurement can help doctors make better treatment decisions and help patients reduce their risk of serious health problems.
