
Many people worry about losing their memory as they grow older, but a new study suggests that some of the most powerful ways to protect the brain may begin decades before symptoms appear.
Researchers found that three simple health goals during middle age were linked to many extra years of life without dementia. The research was published in Neurology Open Access.
Scientists have long known that what is good for the heart is often good for the brain. High blood pressure, diabetes and smoking can damage blood vessels throughout the body. When blood vessels in the brain are affected, brain cells may receive less oxygen and nutrients, increasing the risk of memory problems later in life.
Researchers at NYU Langone Health examined data from the long-running ARIC study, which has tracked thousands of adults since the 1980s. For this analysis they studied 12,409 adults who were about 56 years old on average and had no signs of dementia at the beginning of the research.
The participants were grouped according to whether they had high blood pressure, diabetes or smoked during middle age. Researchers then followed them for roughly 26 years to see who developed dementia and who remained healthy.
The results showed a clear pattern. Adults who had none of these three risk factors enjoyed around 30 years of dementia-free life after joining the study. Those with all three risk factors averaged only 17 dementia-free years, meaning they lost nearly 13 healthy years.
The study also found that women generally lived longer without dementia than men regardless of risk factor levels. White participants also had more dementia-free years than Black participants, suggesting that differences in healthcare access, social conditions and lifelong health may contribute to unequal outcomes.
Lead investigator Dr. Josef Coresh said the findings reinforce the importance of controlling vascular risk factors from about age 45 onward. Preventing smoking, treating high blood pressure early and reducing diabetes risk may all help support healthy brain aging.
The researchers emphasised that the study does not prove these risk factors directly caused dementia because it was observational. It also measured the risk factors only once, meaning later improvements or worsening of health were not captured.
Despite these limitations, the findings fit with many previous studies showing that healthy blood vessels are essential for a healthy brain. Protecting cardiovascular health through regular exercise, healthy eating, maintaining a healthy weight and medical treatment when needed may lower the chances of cognitive decline as people age.
Study review and analysis: One of the greatest strengths of this study is its exceptionally long follow-up, which makes the findings highly relevant to lifelong brain health. However, observational studies always leave open the possibility that other lifestyle or genetic factors contributed to the results. Randomized prevention studies will be needed to confirm how much reducing these risk factors can delay dementia.
