
Many people think of migraine as simply a painful headache, but it is actually a complex neurological condition.
Around one in three people with migraine experience an aura before the headache begins. An aura may cause flashing lights, blind spots, wavy lines or numbness that usually lasts for a short time before the pain starts.
A new study published in Neurology suggests that these warning symptoms could be linked to future stroke risk. Researchers from the University of Vermont found that migraine with aura, but not migraine without aura, was associated with a greater chance of ischemic stroke in adults aged 45 years and older.
The scientists followed more than 11,000 people who had not previously had a stroke. Their average age was 72 years. Participants answered questions about their migraine history, allowing researchers to separate those with aura from those without it.
Over an average follow-up of six years, strokes occurred in several groups. About 3% of people without migraine had a stroke, while approximately 5% of those with migraine with aura experienced one. People with migraine without aura had a stroke rate similar to those without migraine.
When the researchers adjusted for important health factors such as age, diabetes and high blood pressure, they found no increased stroke risk for migraine overall. However, migraine with aura remained linked to a 73% higher risk of ischemic stroke. This suggests that the aura itself may identify people with a higher vascular risk.
One of the most unexpected findings involved male participants younger than 72 years. They had more than a threefold higher risk of stroke if they had migraine, regardless of whether aura was present. This contrasts with previous research in younger populations, where women often appeared to be at greater risk.
The reason for this difference is still unknown. Migraine is believed to involve temporary changes in brain activity and blood vessels, but scientists do not yet know exactly how these processes might influence stroke. Future studies may help identify the biological mechanisms involved.
It is important to remember that this was an observational study. The research cannot show that migraine with aura directly causes stroke, and many people with migraine will never experience a stroke. The results simply indicate that the two conditions occur together more often than expected.
The authors believe their findings may eventually help doctors identify people who could benefit from more intensive prevention strategies. These might include careful control of blood pressure, cholesterol, diabetes and other known stroke risks. Further studies will determine whether migraine history should become part of routine stroke risk assessment.
The study adds valuable evidence that migraine with aura deserves attention beyond headache treatment alone. Its strengths include the large number of participants and detailed statistical analysis, while its main limitation is the inability to determine when migraines first developed.
Overall, the findings support closer monitoring of cardiovascular health in people with migraine with aura, while highlighting the need for additional research before treatment recommendations change.
