Small Everyday Habit May Protect Your Heart

You do not always need a gym, running shoes, or an hour of free time to add useful exercise to your day.

A large research review suggests that an activity many people encounter every day—climbing stairs—is associated with a substantially lower risk of heart-related death.

The study was carried out by researchers from the University of East Anglia and Norfolk and Norwich University Hospital. It brought together data from hundreds of thousands of adults to examine whether taking the stairs could be linked with better long-term health.

The study, “Evaluating the Impact of Stair Climbing on Cardiovascular Risk Reduction: A Systematic Review and Meta-analysis,” was published in the American Journal of Cardiovascular Drugs.

Heart and blood vessel diseases are the world’s biggest killers. They include coronary heart disease, heart attack, stroke, and heart failure, and their risk rises with factors such as smoking, high blood pressure, high cholesterol, diabetes, inactivity, and excess body weight.

Physical activity can reduce several of these risks at the same time. Yet more than a quarter of adults worldwide do not meet recommended activity levels, according to the researchers.

This makes activities that naturally fit into daily routines particularly interesting. Taking the stairs requires no special training and can turn an ordinary trip between floors into a short period of relatively intense movement.

The research team first considered nearly 1,900 studies related to the topic. After assessing the available evidence, they pooled results from nine studies involving more than 480,000 people.

The participants were between 35 and 84 years old, and 53% were women. Some were healthy when the research began, while others already had cardiovascular problems.

The studies followed participants for a median of about 14 years. Researchers then examined whether people who climbed stairs regularly experienced different health outcomes from those who climbed them less often.

People who regularly took the stairs had a 39% lower risk of dying from cardiovascular disease. Their risk of death from any cause was also 24% lower.

The benefits were not limited to mortality. Stair climbing was associated with lower rates of several major cardiovascular conditions, including heart attacks, strokes, and heart failure.

Why might such a simple activity matter? Climbing upward forces large muscles in the legs to work against gravity, quickly increasing the body’s demand for oxygen and making the heart work harder.

Repeated over time, this type of activity can help improve cardiovascular fitness. It can also contribute to stronger leg muscles and may influence blood pressure, cholesterol, blood sugar control, and body weight.

Short bursts may still matter. Previous studies discussed by the researchers found that only a few minutes of stair climbing performed several times a week could improve fitness and reduce cholesterol levels.

One cohort study suggested that roughly six flights per day might be associated with the greatest cardiovascular benefit. But the researchers cautioned that the evidence is not strong enough to prescribe six flights as a universal target.

People differ greatly in age, fitness, health, building access, and physical ability. The amount of stair climbing that is safe and realistic for one person may be too little or too demanding for another.

Dr. Sophie Paddock said the attraction of stairs is their convenience. Instead of setting aside time for a formal workout, someone might take the stairs at work, at a station, or in an apartment building and accumulate activity in small pieces.

This idea reflects a wider change in exercise science. Researchers increasingly recognize that movement accumulated throughout the day can be valuable, even when individual periods of activity are short.

Professor Vassilios Vassiliou said encouraging stair use could become part of wider efforts to reduce cardiovascular disease. Workplaces and public buildings, for example, could make stairways easier and more inviting to use when appropriate.

There is an important caution behind the impressive percentages. The review found an association, which means it cannot prove that taking the stairs directly caused people to live longer.

People who choose stairs regularly may already be healthier or more physically active. They could also have different diets, body weights, smoking habits, incomes, or medical conditions, and researchers cannot perfectly remove all of these differences.

The evidence also came from studies that measured stair use in different ways. Wearable activity trackers could make future research more precise by recording exactly how many flights people climb, how quickly they climb them, and how regularly they do so.

Stair climbing is not the right exercise for everybody. Someone with painful arthritis, balance problems, mobility limitations, or certain cardiovascular conditions may need a safer alternative and should seek professional advice when necessary.

From a research perspective, the size of the analysis is a major strength. Combining data from more than 480,000 people and following participants for many years provides a broad picture of how stair-climbing habits relate to health.

At the same time, the study should not be interpreted as showing that stairs alone can prevent heart disease. Cardiovascular health depends on many factors, including diet, smoking, sleep, body weight, blood pressure, cholesterol, diabetes management, genetics, and overall physical activity.

The findings are best understood as evidence that small daily choices may contribute to a larger pattern of healthy movement. A few flights of stairs cannot replace all other exercise, but they can add activity to a day that might otherwise involve long periods of sitting.

For people who are physically able to use stairs safely, that makes the message refreshingly practical. When both a lift and stairs are available, choosing the stairs from time to time may be a small decision with potentially meaningful benefits over the long term.
