Common Blood Pressure Drug May Raise Higher Risk of Blinding Disease

A common type of medicine used to treat high blood pressure may have an unexpected connection to eye health.

A large UK study found that people taking calcium channel blockers, or CCBs, were more likely to have glaucoma than people who were not using these medicines.

The research was led by Dr. Alan Kastner and colleagues from Moorfields Eye Hospital and University College London. The findings were published in JAMA Ophthalmology, a medical journal that focuses on eye health and disease.

High blood pressure is extremely common, especially among middle-aged and older adults. If it is not controlled, it can increase the risk of heart attack, stroke, kidney disease and other serious health problems, which is why many people need medicine to keep their blood pressure within a healthier range.

Calcium channel blockers are one of the major groups of drugs used for this purpose. They lower blood pressure by relaxing and widening blood vessels, making it easier for blood to move through the body and reducing the workload on the heart.

The researchers examined health information from 427,480 adults in the United Kingdom. Among them, 33,175 people were taking calcium channel blockers.

The team compared people who used these medicines with those who did not. They also took into account differences such as age, lifestyle and other health conditions that might influence a person’s chance of developing glaucoma.

Even after considering these factors, CCB users were more likely to have glaucoma. The researchers did not find the same pattern with other major types of blood pressure medicine, making the possible connection with calcium channel blockers especially interesting.

Glaucoma is a group of eye diseases that damage the optic nerve, which carries visual information from the eye to the brain. The damage often develops slowly, and people may lose part of their vision before they realize that anything is wrong.

Pressure inside the eye is one of the most important known risk factors for glaucoma. When this pressure becomes too high, it can damage the optic nerve over time, although glaucoma can also occur in people whose eye pressure falls within the normal range.

That second point may be important in this study. The researchers found a link between CCB use and glaucoma even though the medicines were not associated with higher pressure inside the eye.

This raises the possibility that calcium channel blockers could influence the optic nerve or other parts of the eye through a different process. Scientists do not yet know what that process might be, and the study was not designed to prove that the medicines themselves cause glaucoma.

Because the research was observational, other differences between people taking CCBs and those not taking them could help explain the findings. More studies are needed to confirm the connection and determine whether certain calcium channel blockers, doses or lengths of treatment carry different risks.

People who take calcium channel blockers should not stop taking them because of this study. Suddenly changing blood pressure treatment without medical advice could be dangerous, and the proven benefits of controlling high blood pressure remain important.

However, the findings may give doctors another reason to pay attention to eye health in people who already have a higher risk of glaucoma. Risk generally rises with age, and family history and certain medical conditions can also make glaucoma more likely.

Regular eye examinations are especially valuable because early glaucoma often causes no obvious symptoms. Detecting the disease before major vision loss occurs gives doctors a better chance to slow its progression with treatments such as eye drops, laser procedures or surgery.

For now, the study highlights an important question rather than providing a final answer. Researchers hope further work will reveal whether calcium channel blockers directly affect glaucoma risk and whether some patients might benefit from closer eye monitoring while using these widely prescribed medicines.
