A Simple Blood Pressure Mistake That Could Cause the Wrong Diagnosis

Checking blood pressure is one of the easiest and most important ways to protect heart health.

Doctors use these numbers to decide whether someone has high blood pressure, which is a major risk factor for heart attacks, strokes, kidney disease, and other serious health problems.

Because high blood pressure often causes no warning signs, millions of people depend on accurate blood pressure tests to detect the condition early.

A new study from Johns Hopkins Medicine has found that a very small detail during a blood pressure check can make a surprising difference.

The researchers discovered that the position of a person’s arm can change the reading enough to make blood pressure appear higher than it really is. The findings were published in Cerebral Circulation–Cognition and Behavior.

Health experts recommend sitting in a chair with the back supported, both feet flat on the floor, and the arm resting on a desk or table at the same height as the heart. This position helps provide the most reliable reading.

However, this is not always how blood pressure is measured in busy clinics or medical offices.

Many people have their blood pressure checked while sitting on an examination table without proper support. Others rest their arm on their lap or leave it hanging by their side. In some cases, a healthcare worker supports the arm by hand, which may also affect the result.

To understand how much arm position matters, the Johns Hopkins researchers studied 133 adults from different backgrounds.

During a single visit, every participant had blood pressure measured using three different arm positions. Before each set of measurements, participants walked for two minutes, similar to what many people do before arriving in an examination room, then rested before having three blood pressure readings taken with a digital monitor.

The researchers found that resting the arm on a desk produced the most accurate readings. When the arm rested on the lap, the average systolic blood pressure, the top number, was almost 4 mmHg higher. When the arm hung unsupported at the side, the systolic reading was nearly 7 mmHg higher than it should have been.

The lower number, known as diastolic blood pressure, was also affected. Both the lap position and the unsupported arm position increased the bottom number by about 4 mmHg. Although these changes may seem small, they can be large enough to change a person’s diagnosis.

For example, blood pressure guidelines use specific cut-off values to decide whether treatment may be needed. A reading that is only a few points too high could incorrectly label someone as having hypertension. This could lead to unnecessary worry, extra medical visits, or even medication that may not have been needed.

The study highlights why correct measurement techniques are just as important as having a good blood pressure monitor. While this research focused on digital blood pressure devices, the message is relevant for anyone who checks blood pressure regularly. Accurate testing helps doctors make better decisions and gives patients a clearer picture of their health.

If you measure your blood pressure at home, remember to sit quietly for a few minutes beforehand, keep both feet flat on the floor, support your back, and rest your arm on a table so it is level with your heart. If you are having your blood pressure checked in a clinic, it is reasonable to ask for your arm to be supported correctly if it is not already.

A simple change in arm position could prevent an incorrect reading and help ensure you receive the right care.
