What Are the Common Causes of Strokes?

A stroke happens when blood flow to part of the brain is suddenly blocked or when a blood vessel in the brain bursts. Without enough oxygen and nutrients, brain cells begin to die within minutes.

Stroke is one of the leading causes of death and long-term disability around the world, but research has shown that many strokes can be prevented by understanding and reducing the factors that trigger them.

There are two main types of stroke. The most common is an ischemic stroke, which is caused by a blood clot blocking an artery that supplies the brain.

The second type is a haemorrhagic stroke, which happens when a weakened blood vessel breaks and bleeds into the brain. A transient ischemic attack, often called a mini-stroke, is a temporary blockage that can be an important warning sign of a future stroke.

The strongest trigger for stroke is high blood pressure. Large international studies have consistently shown that uncontrolled blood pressure damages blood vessels over many years, making them more likely to become blocked or rupture. Lowering blood pressure through healthy lifestyle changes and medication is one of the most effective ways to reduce stroke risk.

Smoking is another major trigger. Chemicals in cigarette smoke damage blood vessels, increase inflammation and make blood more likely to clot. Research shows that quitting smoking reduces stroke risk, and the benefits begin within months of stopping.

Diabetes is also closely linked to stroke. High blood sugar gradually damages blood vessels throughout the body, including those supplying the brain. Good blood sugar control, regular exercise and following medical advice can significantly reduce this risk.

High cholesterol contributes by allowing fatty deposits, known as plaque, to build up inside arteries. Over time these deposits can narrow blood vessels or break open, causing a clot that blocks blood flow to the brain. Clinical research has shown that cholesterol-lowering medicines such as statins reduce the risk of stroke in many people at increased cardiovascular risk.

An irregular heartbeat called atrial fibrillation is another common trigger. Instead of pumping blood smoothly, the upper chambers of the heart beat unevenly, allowing blood clots to form. If one of these clots travels to the brain, it can cause a stroke. Blood-thinning medicines greatly lower this risk for many patients.

Obesity and physical inactivity also increase stroke risk. Large population studies show that regular physical activity improves blood pressure, cholesterol, blood sugar and overall cardiovascular health. Even moderate activities such as brisk walking can make a meaningful difference when done regularly.

Diet also matters. Research supports eating patterns rich in vegetables, fruit, whole grains, legumes, fish, nuts and olive oil, such as the Mediterranean diet. These foods help reduce inflammation and support healthy blood vessels, while diets high in salt, added sugar and highly processed foods increase cardiovascular risk.

Heavy alcohol use can trigger stroke by raising blood pressure and increasing the chance of irregular heart rhythms. Drinking within recommended limits or avoiding alcohol altogether may reduce this risk.

Long-term stress and poor sleep may also contribute. Research suggests that chronic stress can raise blood pressure and encourage unhealthy behaviours such as smoking, overeating and physical inactivity. Sleep disorders, including obstructive sleep apnoea, have also been linked with a higher risk of stroke.

Certain medical conditions, including kidney disease, inflammatory diseases and some inherited clotting disorders, may further increase risk. Doctors often recommend additional monitoring and treatment for people with these conditions.

Although some risk factors such as age and family history cannot be changed, research shows that most strokes are linked to factors people can improve. The INTERSTROKE study, one of the largest international stroke studies, found that a small number of modifiable risk factors account for the great majority of strokes worldwide.

Recognising the warning signs of stroke is just as important as preventing it. Sudden weakness on one side of the body, facial drooping, trouble speaking, loss of vision, severe dizziness or a sudden severe headache require immediate emergency medical care. Early treatment can save brain tissue and greatly improve recovery.

Overall, research shows that stroke usually develops after years of damage to blood vessels rather than from a single event.

Controlling blood pressure, avoiding smoking, managing diabetes and cholesterol, staying physically active, maintaining a healthy weight, eating a balanced diet and following medical advice remain the most effective ways to reduce stroke risk and protect long-term brain health.
