One Simple Pill Could Help Prevent Thousands of Heart Attacks and Strokes

Heart disease is the leading cause of death around the world. It increases the risk of heart attacks, strokes, and heart failure, which can shorten lives and reduce quality of life.

Many of these problems are linked to high blood pressure, high cholesterol, smoking, poor diet, lack of exercise, and diabetes.

Doctors often prescribe several different medicines to lower these risks. One medicine may reduce blood pressure, another lowers cholesterol, and another helps prevent blood clots. Although these treatments work well, remembering to take several pills every day can be difficult, and missing doses can reduce their benefits.

A research team from Tehran University of Medical Sciences investigated whether combining several common heart medicines into a single tablet could make treatment easier.

This combined medicine, known as a polypill, contains two blood pressure medicines, one cholesterol-lowering medicine, and a low dose of aspirin. The idea is that one daily pill may improve both convenience and long-term treatment success.

The researchers tested the polypill in a large study involving about 6,800 adults between 50 and 75 years of age in Iran. Some participants had already experienced heart disease, while others had not. Everyone received advice about healthy eating, regular physical activity, and other lifestyle changes, but only half of the participants took the polypill every day.

The polypill contained 81 milligrams of aspirin, 20 milligrams of atorvastatin to lower cholesterol, 12.5 milligrams of hydrochlorothiazide to reduce blood pressure, and 5 milligrams of enalapril, another blood pressure medicine. All of these medicines have been used safely for many years and are already widely available around the world.

The study followed participants for five years. During that time, about 6 out of every 100 people taking the polypill experienced a major heart-related event, compared with about 9 out of every 100 people who did not receive the combined pill. This translated to roughly a 34% lower risk of serious cardiovascular problems.

After accounting for the use of other medicines, the researchers still found that the polypill lowered the risk of major heart problems by about 22%. People who took the medicine regularly without missing doses received the greatest benefit, highlighting how important long-term adherence is for preventing heart disease.

The researchers also found that the cholesterol-lowering medicine appeared to contribute the largest share of the overall benefit. Lowering blood pressure remained important, but controlling cholesterol may have had the strongest effect in reducing the risk of blocked arteries that can lead to heart attacks and strokes.

A combined pill like this could be especially valuable in countries where healthcare resources are limited. A single affordable medicine may reduce costs, simplify treatment, and help more people continue taking their medicines as prescribed. Even in countries with well-developed healthcare systems, some patients may also benefit from having a simpler treatment plan.

The study was led by researchers at Tehran University of Medical Sciences and showed that combining several proven medicines into one daily pill could make preventing heart disease easier and more effective.

Although doctors still need to decide which patients are most suitable for this approach, the findings suggest the polypill could become an important tool for reducing heart attacks and strokes, particularly in communities where access to healthcare is limited.
