Study shows big benefits of stricter high blood pressure control

A new study has found that keeping blood pressure at a lower level than usual can greatly reduce the risk of serious heart problems. This is especially important for people who already have a high risk of heart disease.

High blood pressure, also called hypertension, is very common around the world. It happens when the force of blood pushing against the walls of the blood vessels is too strong. Over time, this extra pressure can damage the heart and blood vessels. It increases the risk of heart attacks, strokes, heart failure, and even death.

Blood pressure is measured using two numbers. The top number is called systolic blood pressure. It shows how much pressure there is when the heart beats. For many years, doctors have aimed to keep this number below 140 mm Hg for most patients.

However, new research suggests that aiming even lower could bring greater health benefits.

The study, called the Effective Systolic Pressure Reduction Intervention Trial, or ESPRIT, was carried out in China. Researchers wanted to find out whether lowering systolic blood pressure to below 120 mm Hg would provide better protection against heart disease compared to the usual target.

They studied people who were at high risk of heart problems and divided them into two groups. One group received standard treatment, aiming for blood pressure below 140 mm Hg. The other group received more intensive treatment, aiming for a lower target below 120 mm Hg.

The results showed clear benefits for the group with stricter control. People in this group had a 12% lower risk of major heart-related events. These events included heart attacks, strokes, deaths caused by heart disease, hospital stays for heart failure, and procedures to improve blood flow in the heart.

Even more striking was the effect on survival. The intensive treatment group had a 39% lower risk of dying from heart-related causes compared to the standard treatment group. This is a large reduction and suggests that better blood pressure control can save many lives.

The study also found that deaths from all causes were reduced by 21% in the intensive treatment group. This means that lowering blood pressure more carefully may not only protect the heart but also improve overall health and longevity.

One important question was whether lowering blood pressure this much could cause harm. If blood pressure drops too low, it can lead to dizziness, fainting, or problems with the kidneys.

The researchers carefully monitored these risks. They found that serious side effects were not much higher in the intensive treatment group. There were no major increases in problems such as very low blood pressure, serious changes in body salts, kidney damage, or injuries from falls.

There was a small increase in fainting episodes, also known as syncope, but this was rare. Overall, the benefits of stricter blood pressure control appeared to outweigh the risks for most people in the study.

This research is important because high blood pressure is one of the leading causes of disease worldwide. Many people live with hypertension without knowing it, and others may not have it well controlled. Finding better ways to manage blood pressure could prevent a large number of serious health problems.

The findings suggest that aiming for a lower blood pressure target could become a new strategy in preventing heart disease, especially for people at high risk. However, treatment should always be personalized. Not everyone may need such a low target, and decisions should be made together with a doctor.

In the end, this study highlights the power of prevention. By managing blood pressure more carefully, it may be possible to reduce heart disease, save lives, and improve overall health.
