A simple pill may reduce high cholesterol for people who can’t use statins

Cholesterol is a soft, waxy substance that is found in every part of the body. It is not always harmful. In fact, the body needs cholesterol to build healthy cells, make hormones, and help digestion.

The liver produces most of the cholesterol we need, but we also get some from foods, especially those that come from animals such as meat, eggs, and dairy products. Problems begin when there is too much cholesterol in the blood, especially a type called low-density lipoprotein, or LDL, which is often known as “bad” cholesterol.

When LDL levels become too high, the extra cholesterol can slowly build up inside the walls of blood vessels. Over time, this buildup forms a sticky layer called plaque. As plaque grows, it narrows the arteries and makes it harder for blood to flow smoothly.

This condition is called atherosclerosis. If a blood clot forms or a plaque suddenly breaks, it can block blood flow completely. This can lead to serious health problems such as heart attacks and strokes. These conditions remain some of the leading causes of death around the world, which is why controlling cholesterol is so important for long-term health.

For many years, the most common way to treat high cholesterol has been with a group of medicines called statins. These drugs work by reducing the amount of cholesterol made by the liver. For many people, statins are very effective. They can lower LDL levels significantly and reduce the risk of heart disease.

However, not everyone can take statins. Some people experience side effects such as muscle pain, joint discomfort, or fatigue. In some cases, these side effects are mild, but for others, they can be strong enough to make daily life uncomfortable. There are also people whose cholesterol levels do not drop enough even when they take statins regularly.

It blocks a step in the process the body uses to make cholesterol, but it acts mainly in the liver and not in muscles. This may explain why it causes fewer muscle-related side effects for some people.

The benefits of bempedoic acid were studied in a large research project known as the CLEAR Outcomes Trial. This study focused on people who could not tolerate statins or could not take them at full doses.

Researchers wanted to see whether this new drug could still lower cholesterol and reduce the risk of heart problems in these patients. The findings were presented at the Endocrine Society’s annual meeting in Chicago in 2023 and later discussed widely in the medical community.

The results were encouraging. People who took bempedoic acid saw their LDL cholesterol drop by about 21 percent on average. More importantly, the risk of major heart problems, including heart attacks and strokes, was reduced by about 13 percent. These results showed that lowering LDL cholesterol with this new medicine can still provide real protection for the heart.

Dr. A. Michael Lincoff from the Cleveland Clinic, one of the lead researchers in the study, highlighted the importance of these findings.

He explained that heart disease continues to be the leading cause of illness and death worldwide, and there is a strong need for more treatment choices. Having another effective option can help doctors better support patients who cannot rely on standard treatments like statins.

What makes this discovery especially important is that not all medicines that improve one health measure also improve heart outcomes.

For example, some diabetes drugs lower blood sugar but do not always reduce the risk of heart disease. In contrast, both statins and bempedoic acid appear to protect the heart in a similar way by lowering LDL cholesterol, which is a key driver of artery damage.

This new option may offer hope to many people who have struggled to manage their cholesterol levels. It can be used alone or together with other treatments, depending on each person’s needs. As research continues, doctors will learn more about how to use this medicine in the best way to help patients live longer and healthier lives.

Overall, the development of bempedoic acid shows how medical research continues to improve the way we prevent and treat serious diseases.
