Two common blood pressure drugs have surprising side effects

A 17-year study from Columbia University has brought new insights into the long-term effects of two commonly used blood pressure medications: chlorthalidone and hydrochlorothiazide.

These medications are widely prescribed to manage high blood pressure and prevent heart-related issues like heart attacks, strokes, and hospitalizations for heart failure.

While both drugs are effective in lowering blood pressure, the study reveals important differences in their safety, which may impact which one is a better choice for certain patients.

Over the course of the study, researchers followed more than 730,000 people who were being treated for high blood pressure. They discovered that chlorthalidone, despite being effective, had more significant side effects compared to hydrochlorothiazide.

The biggest concern was the higher risk of a condition called hypokalemia, which is when potassium levels in the blood drop to dangerously low levels.

Low potassium can cause serious health problems, including irregular heartbeats, confusion, kidney problems, and in some cases, the development of type 2 diabetes.

According to the data, about 6.3% of patients taking chlorthalidone experienced hypokalemia, compared to only 1.9% of those taking hydrochlorothiazide. This threefold increase in risk raised concerns, even when chlorthalidone was used in smaller doses.

Additionally, chlorthalidone was linked to other issues, such as electrolyte imbalances and kidney problems, which further complicate its use in treating high blood pressure. These side effects can be severe, sometimes leading to kidney failure or abnormal heart rhythms.

The study’s findings suggest that hydrochlorothiazide might be a safer option for many patients, given its lower risk of these complications. This is an important finding because many treatment guidelines currently prefer chlorthalidone as the first-choice medication for high blood pressure.

However, the results of this research could lead to a reevaluation of those recommendations, especially for patients at risk of developing the side effects associated with chlorthalidone.

For patients and doctors, this study highlights the need for careful consideration when choosing a blood pressure medication. While it is essential to lower blood pressure to prevent life-threatening conditions, the choice of drug must also take into account the potential risks.

Close monitoring of patients on chlorthalidone, especially for potassium and electrolyte levels, may be necessary to prevent these dangerous side effects.
